An exciting opportunity has arisen for a multidisciplinary Trainee Advanced Clinical Practitioner (TACP) at Band 7 to join our Urgent Community Response Services Directorate within the Primary Care, Community, and Therapies Clinical Group (PCCT).
Whether you are beginning your TACP journey or partway through your MSc in Advanced Practice and considering a move into Urgent Community Response services, this role offers supportive and progressive environment to continue your development.
You'll work within a skilled and experienced team contributing to the Department of Health and Social Care (DHSC) 10-Year Health Plan for England, focusing on key NHS priorities and the Three Major Shifts.
You'll collaborate with a multi professional ACP team, trainee ACPs, enhanced practitioners, community matrons and case managers.
Our Trust has a strong understanding of advanced practice. The directorate is supported by our Lead ACP, working closely with the organisation's Director of Advanced Practice to ensure robust supervision, governance, and delivery across all four pillars of advanced practice.

Main duties of the job
You'll be expected to successfully complete the MSc Advanced Clinical Practice (Level 7) programme in line with the Trust's advanced practice framework and your professional regulatory body.
The MSc programme will support your development and mapping against the Virtual Ward and Urgent Community Response Capabilities Framework (2022) as part of the NHS ePortfolio.
The TACP role, forms an 85:15 split between clinical practice and protected study time to support academic learning, skills development, and progression throughout the programme.
As a TACP, you'll build on your existing clinical experience to manage patients within the Urgent Community Response (UCR) setting. Working closely with your clinical mentor, you'll learn to undertake patient-focused assessments, manage a range of medical presentations, and request or perform appropriate investigations within the community environment.
Over time, you will be expected to work autonomously within recognised clinical situations.
With training, you will begin to understand and apply the principles of therapeutics and safe prescribing, working within legal and professional frameworks for advanced practice.
With the support of the MSc programme and following completion of all required Trust competencies, the trainee ACP will progress to a Band 8a Advanced Clinical Practitioner role at the end of the agreed training period.

This will encompass training in the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice in defined clinical areas of the group. The trainee Advanced Clinical Practitioner (tACP) will develop specialist knowledge and skills to provide healthcare autonomously to patients in their specialty through an established ACP training pathway.
The post holder will be required to undertake and complete a programme of education to achieve a Masters in Advanced Clinical Practice via apprentice route as the preferred option. The TACP will follow the co-ordinating educational supervisor supported ACP training program mapped against established national and regional competencies.
