Main duties of the job
To be a high profile, accessible and responsive leader with the responsibility andaccountability for the provision, management and development of a qualityspecialist Anticoagulant nursing service that meets the needs of the patients inaccordance with national and local policies and initiatives.To assist the Matron/Group Director of Nursing to develop and implement effectivesystems and working practices to facilitate service developments within thedepartment and encourage team members to embrace best practice and newmodels of working.Set high professional standards as the clinical leader for all department staff. Byensuring that all staff have the adequate skills and competence to deliver highquality and clinically effective patient care.The post holder will act as an independent practitioner using advanced clinical skillsto assess, diagnose, treat, prescribe plans of care and mange caseloads of patientsThey will also need to be able to make informed decisions within their own areaand scope of professional practice, involving patients and public to ensureagreed clinical standards and targets are met.To manage and co-ordinate staff and clinical activity within the ward/department,playing a key role within the multi-disciplinary team.Take the lead role for department change management activity, developing andmaintaining strong communication links with divisional operational managementand others.

- Lead the development of the Anticoagulation Nursing Service, ensuring care is patient-centred and evidence-based.
- Demonstrate advanced clinical knowledge and innovative practice within dermatology and anticoagulation.
- Act as a role model, delivering high standards of holistic care and clinical leadership.
- Complete prescriptions according to treatment plans and clnical indication
- Accept direct patient referrals, perform differential diagnoses, and coordinate care with consultants.
- Support patients and families facing emotional challenges, referring to Clinical Psychology as appropriate.
- Initiate discharges and transfers to specialist centres or other care settings.
- Develop nurse-led clinics and specialist protocols aligned with service needs.
- Provide expert clinical advice to patients, carers, GPs, and community nurses.
- Identify and meet the educational needs of patients and carers.
- Attend and present at weekly multidisciplinary team (MDT) meetings.
Managerial Responsibilities - Provide clinical leadership across the organisation, supporting junior staff and conducting appraisals.
