Job summary

An exciting opportunity for a multiprofessional Advanced Clinical Practitioner(ACP) Band 8a, has arisen to join the Urgent Community Response Services Directorate within the Primary Care, Community and Therapies Clinical Group (PCCT).

Aligned with the Department of Health and Social care (DHSC) 10-Year Health Plan for England, this role supports key National priorities and Three Major Shifts, contributing to essential workforce, delivering;

  • Hospital to Home: Expanding care in community settings to improve access and reduce hospital pressures.
  • Shifting Care Closer to Home: Increasing community-based services to minimise reliance on acute care.

Recognising the importance of continued professional development and pivotal role ACPs play in shaping future services, our Lead ACP, working closely with the Director of Advanced Practice, ensures robust supervision, governance and development across four pillars of advanced practice.

You will collaborate with ACPs, tACPs, ENP, community matrons and case managers.

Our Trust is the first in the Midlands to establish a true Care Navigation Centre, helping residents access emergency, urgent, and non-urgent community services. This innovative model of multiprofessional workforce, work to a "no wrong door" ethos, ensuring patients reach the right service first time.

Main duties of the job

o To work as an autonomous practitioner utilising advanced theoretical and practical knowledge.o Assess patients presenting with acute illness or for review of chronic disease conditions.o Undertake home visits, including care homes, for acute illness or chronic disease management.o Act as senior clinical decision makers on intermediate and community care in-patient wards, with appropriate medical support.o Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.o Clinically examine and assess patient needs from a physiological and psychological perspective, and plan and implement clinical care accordingly.o Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).o Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies.o Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.oUtilise and demonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment.o Demonstrate a high level of verbal and nonverbal communication skills to enable full and details patient assessments are achieved.
