Community Emergency Care/Nurse Practitioner

Job summary

An exciting opportunity has arisen for an experienced Emergency Practitioner with a background in Nursing or Paramedicto join the Urgent Community Response Services at Sandwell and West Birmingham NHS Trust.

The post holder will be a registered Nurse, Paramedic, or Allied Health Professional who, acting within their professional boundaries, will deliver comprehensive care to referred patients. This will include conducting initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.

They will demonstrate safe and effective clinical decision-making and provide expert care for patients within the Urgent Community Response Service. The post holder will work collaboratively with the multi-disciplinary Community and Primary Care Network Team to address the needs of patients effectively and holistically.

On successfully completing a 12 month period and trust ePortfolio, mapped to the Virtual Ward and Urgent CommunityResponse Capabilities Framework, the successful candidate will progress to a Trainee Advanced Clinical Practitioner role, as part of their ECP/ENP role over 3 years, at the next available cohort of MSc at the approved Higher Education Institution, through an apprenticeship/NHSE funded route.

The post holder will primarily be based at Sandwell Council House.

Main duties of the job

The post holder will be a registered Nurse, Paramedic, or Allied Health Professional who, acting within their professional boundaries, will deliver a high standard of patient care as an Emergency Practitioner within the Urgent Community Response (UCR) Service.

Using enhanced autonomous clinical skills and a broad, in-depth theoretical knowledge base, the post holder will:

  • Assess and triage patients, including providing a two-hour response when appropriate.
  • Deliver definitive treatment, including prescribing medications in line with policies, Patient Group Directives (PGDs), NICE (National Institute for Health and Care Excellence) guidelines, local clinical protocols, and care pathways.
  • Make referrals to appropriate members of the primary, community, or acute care teams when necessary.
  • Critically evaluate and analyse complex clinical problems, applying expertise and knowledge to inform decision-making.
  • Consult with colleagues to devise and evaluate complex care plans tailored to patients' specific needs within their field of expertise.

This role requires a proactive approach to patient care, with the ability to work collaboratively across multidisciplinary teams while maintaining the highest clinical standards.

  • Educated to a degree level in Nursing.
  • Has completed consolidation of learning period as a newly qualified Nurse
  • Nursing & Midwifery Council (NMC) registration
  • Post Graduate level qualification or equivalent experience relevant experience e.g. Emergency Department, UTC, UCC or GP Practice qualification
