Job summary

We have an exciting opportunity for a band 5 or 6 nurse who wants to further develop their skills within cardiology and heart failure to join our Acute Heart Failure Nursing Team, on a 1 year fixed term contract to cover maternity leave.

The heart failure pathway is multi-faceted and involves admission avoidance strategies, in-patient advice and management, facilitated discharges and arranging community heart failure care where appropriate.

The patient journey encompasses a variety of transitions; therefore the ideal candidate requires both excellent clinical and communication skills. The job is rewarding and there are lots of opportunities to make a difference to the heart failure patient journey.

This roles offers great development for anyone with an interest in heart failure/cardiology. With close mentoring from the heart failure multidisciplinary team and the opportunity to work collaboratively within the wider Cardiology team in both primary and secondary care.

We are looking to recruit an experienced, dynamic and compassionate nurse, to not only plan and deliver holistic care across all phases of heart failure from diagnosis to end of life, but also assess and plan care for acutely unwell patients with suspected heart failure.

The post will involve cross site working and some weekend and Bank Holiday working.

Main duties of the job

If appointed, you will be expected to play a key role in overseeing the day to day cross-site management of acute heart failure patients, assessing and managing heart failure patients in both the acute and out-patient clinic setting.

Applicants should be committed to delivering high standards of care, be able to work well as a team member but also have the confidence to work independently without direct supervision.

The successful candidate will need to be confident and dynamic, working with all levels of the MDT across secondary and primary care.

We can offer informal study opportunities to enable the successful candidate to develop knowledge and skills in this specialist area.
