Job summary

An exciting opportunity has arisen for an ACP ( Advance Clinical Practitioner) Lead at Band 8b to be part of a multidisciplinary Urgent Community Response Services Directorate within the Primary Care, Community and Therapies Clinical Group. Leading on the development of our ACPs, tACP and other advanced practice, primarily within the UCR directorate but also across Primary Care and broader community services all of which are integral to our Trusts commitment to deliver excellent and responsive integrated care. The successful candidate will have the opportunity to contribute to the group's delivery of the trusts' strategic ambition of delivering for our People, Patients and Population. We are the first in the Midlands to develop a true Care Navigation Centre, supporting the local population to access emergency care, urgent and non-urgent community services, untilsing both clinical and adminstrative members of our MDT. With your help we would continue to be the first in many other endeavours, whilst maintaining our ethos of "no wrong door" and right patient, right team, first time.

You will be working within a friendly and supportive team and be part of an expanding new and exciting Directorate, incorporating UCR, Virtual Wards and Single Point of Access & front door streaming.

Main duties of the job

The provision of highly specialised advice, consultancy and clinical assessments at expert level.

They will provide clinical leadership and support for the ACPs and tACPs across Primary Care, Community and Therapies group. The post holder will be responsible for service development, guidance, motivation, governance, and clinical standards as well as operational oversight of the ACPs across the directorates.

They will support and play a key role in the development and supervision of advanced practice at a local, Group, Trust and regional level, supporting the trust Director of ACP and the chief AHP (in the strategic and operational development and delivery of ACP services across the directorate and group).

Supports the Trusts Director of ACP in the implementation of the long-term ACP strategic plan.
