Job summary
An exciting opportunity has arisen for us to recruit community focused and creative Band 7 Community Matrons for our iCares Community Team. The locality Team support people to manage their long term conditons and avoid hospital admissions where possible. They will work as part of a wider integrated community team to support people holistically living in their own homes.
You will be supported by the wider Integrated Care Service (iCares) which works across Sandwell Community providing rehabilitation, case management and admission avoidance.
There is the opportunity to bring your leadership and clinical skills to help guide our evolving services as well as being part of developing the new integrated Town Teams model.
Main duties of the job
The key duties for the post holder will be;
To provide leadership and oversight of the team working across community.
To be a lead clinician within the service, co-ordinating, delivering and evaluating the care provided to patients on a daily basis.
To provide highly developed specialist assessment and treatment of patients with planned and unplanned complex presentations within community settings.
To supervise, assess and educate team members including bank or agency staff, support workers and students.
