Job summary
This is a new and exciting job role working within the Sandwell Home Based Intermediate Care team (HBIC) supporting patients efficiently within their own homes.

We are looking for a B3 Healthcare Assistant who believes in the value of quick intervention and nursing support to maintain peoples safety and independence at home.
The HBIC team works as part of a wider integrated Directorate (iCares) working with people at home who have mobility or other healthcare needs. This includes intensive community rehab, nursing support and admission avoidance delivered by a multi-disciplinary team.
We work closely with our colleagues in Urgent Community Response, Locality teams and Virtual Wards, which means working closely with the wider Multi-disciplinary Team
Our team work closely with patients to support and manage issues at home where possible and safe to do so. You will be supporting nurses and the wider MDT to achieve best possible patient centred outcomes.Your line manager will be one of the nurses in the team and no one works in isolation. You will receive support and supervisions from your line manager who will be based within the team.
Due to current service needs, we are only able to accept those who have a NVQ Level 3 Healthcare or Equivalent
Main duties of the job
- Provide the intensity healthcare support for people in the community.
- To work closely and collaboratively with colleagues from the wider team, local authority and third sector.
- To support the HBIC team in assessing and supporting patients in need of support in the community.
- To bring value and compassion into everything you do.
