Job summary
The post will be part of a well-established Vulnerable Adult Team. working towards an Integrated Safeguarding Adults and Children Team. The post holder will be highly visible across clinical areas, working with multidisciplinary teams in an acute hospital setting. Thepost holder will be part of a liaison service providing specialist support to patients living with Learning Disabilities and or Autism, their carers throughout their inpatient pathway using proactive case load management.
Main duties of the job
Working as part of the Safeguarding and Vulnerable Adult Team to support service users, carers and staff to improve patient experience and hospital journeys. Providing an expert point of contact, providing Learning Disability/Autism training. Supporting staff to adhere to statute duties.

The post holder will be required to:
- Act, in accordance with the framework or scope of professional practice Management ofcases after initial screening
- Specialist assessment and early intervention, including the assessment of psychosocial needs and risk management
- Improve access to assessment, treatment, advice and support
- Deliver specialist care and support clinical teams to develop hospital plans
- Promote hospital passports and Johns Campaign
- Supporting teams with the development and implementation of individualised care plans inclusive of appropriate observation, reasonable adjustments and management ofdistressed behavior.
- Signpost service users and support early referrals to community services/specialistteams and health facilitation
- Support and appropriate referral for carers of patients with Learning Disabilities.
- Early response to both planned and unplanned admissions
- Maintain a clinical database and dashboard. Use the information to contribute to reports, benchmarking and audits as required
- To promote and comply with NMC professional standards and local standards for recordkeeping.
- Be professionally accountable for all professional and personal.
- Promote effective communication.
- Provide Teaching sessions to improve Learning Disability/Autism standards.
