This post is for a qualified ACP with a passion for primary care. You will bring your clinical expertise to a wider multidisciplinary team.

Clinical support and supervision are available for all staff.

Main duties of the job

To be a clinical expert and senior clinician in the field of Primary Care.

To work with the PCN Lead ACP and wider team to continually improve the quality of care provided including the continuation of service improvement to demonstrate better outcomes for our patients.

Your Health Partnership (YHP) is a cutting-edge Primary Care Network working as part of Sandwell and West Birmingham Hospitals Trust.

The successful applicant will need to share the same values of YHP and work as part of a large multi-disciplinary team including GPs, Nurses, Clinical Pharmacists and Administrators.

  • MSc level qualification in advanced clinical practice
  • Master's level education in relevant health related subject
  • Completion of recent Leadership /Management or evidence of equivalent competence Evidence of contemporaneous CPD
  • Independent Prescriber qualification or working towards

Desirable

  • Leadership / management course.
  • Evidence of chronic disease management

Experience

Essential

  • Significant recent experience in a senior nursing or allied health professional role within a primary care or emergency care setting
  • Experience of leading clinical teams and professional agendas
  • Experience of leadership role and change management
  • Proven track record in improving patient experience and gaining patient/public views
  • Recent experience of direct involvement in delivering the Quality Agenda
  • Experience of telephone triage
  • Experience of effective interagency working

Desirable

  • Experience in clinical systems e.g. Systmone
  • Experience of multiskilled approach to Chronic Disease Management
  • Evidence of innovative practice
  • Experience of working within an ACP post within primary or community care
  • Experience of working as or towards autonomous practice
