Job summary

An exciting opportunity has arisen to join Sandwell and West Birmingham Hospitals as a Band 3 Patient Flow Assistant within Acute Medicine.

The role is new and as such allows the successful candidates to develop personally and professionally.

The role will be based cross site at Sandwell General Hospital and City Hospital, with rotation across the two sites expected. During Spring 2024, the organisation will move to the new Midland Metropolitan University Hospital in Smethwick, which will transition the team to single site working.

Shift Patterns are as follows:

  • Long Days 07:30 - 19:30
  • Monday to Sunday
  • 34.5 Hours per Week - 3 Long Days
  • 23 Hours per Week - 2 Long Days

Main duties of the job

The purpose of the Patient Flow Assistant (PFA) role is to ensure that patients have a planned timely transfer or discharge, and that the patient's pathway is managed, ensuring all patients have a positive experience of the hospital admission or discharge process.

The PFA will be based on the Acute Medicine Unit supporting the Clinical Shift Lead/Nurse in Charge/Unit Lead to ensure that transfer or discharge planning is pro-active and proceeds without unnecessary delay.

It is also expected that the PFA will work closely with the Medical, Nursing and Support Service Staff to ensure that patients are proactively managed through the system, checking that elements of the patients pathway such as investigations, medications and transport are being requested in a timely way.
