Clinical Trainer- Community Children’s Nurse

Job summary
We are looking for a motivated, experienced nurse to join our well-established Community Childrens Complex Care Nursing Team. The post holder will provide specialist nursing skills knowledge and expertise in the care and management of children and young people who have a confirmed continuing health care need.
As a Senior CCN you will have the opportunity to shape and lead our responsive service as it continually adapts to the ever-changing needs of the families we care for.
You will have a key role in leading care planning, training and multi-disciplinary liaison for CYP requiring a complex care package across a range of settings including home, school and wider community settings.
The postholder willneed to work autonomously using excellent communication and decision-making skills to be able to manage complex anddifficult situations in a sensitive manner.
Experience in working in this environment is desirable.
You will be part of the wider Community Childrens Nursing team with a high level of nursing expertise and be a role model to the nurses and students within the team.
The service operates 7 days a week, 24 hours per day.

Main duties of the job
To work in partnership with child, family and carers; holistically in response to their specific needs, maximizing their independence and quality of life.
To co-ordinate patient care pathway and liaise across organisational boundaries
To support continuity of care within an integrated care organisation following the patient across the acute and community pathways.
To lead on the teaching and development of healthcare professionals, children, young people and carers.
To provide clinical supervision and mentorship for junior staff, health care assistants as required.
The post holder will
RN Child Branch/ RSCN
- Three years post registration experience
- have acute and preferably community experience.
- demonstrate good written and oral communication skills
- have a current driving licence and access to a car
