Job summary
Weald Primary Care Network is seeking an experienced and motivated Band 7 Mental Health Practitioner to join our growing Primary Care Mental Health MDT, supporting patients across 8 GP practices in the Weald, although predominantly working from one practice in Goudhurst. This is a key senior role, working closely with GPs, allied health professionals and community partners to deliver timely, high-quality mental health support for patients with mild to moderate mental health needs, including anxiety, depression and functional mental health difficulties. The post holder will provide autonomous clinical input, contribute to holistic assessment and care planning, and support the delivery of person-centred, recovery-focused interventions within a primary care setting.The successful candidate will bring a background in mental health nursing, occupational therapy or a related profession, with strong clinical judgement, confidence in risk management, and experience of working collaboratively within multidisciplinary teams. As a senior practitioner, you will play an active role in MDT development, offering advice and guidance to colleagues, including our Health and Wellbeing Coach, supporting pathways into NHS Talking Therapies and community services, and helping to reduce GP workload through effective mental health input in primary care
Main duties of the job
This is an excellent opportunity to shape and influence mental health provision across the PCN while working in a supportive, forward-thinking environment focused on integration, quality and patient outcomes.
The role requires a highly competent, proficient practitioner, who is able to work autonomously and creatively in this busy and demanding role. Excellent assessment skills and knowledge of local services are essential to this role. Experience of being in a leadership role is valuable.
You will also be providing supervision to band 6 Mental Health Practitioners.
Successful candidates must be experienced Mental Health Practitioners and have experience of working in a Primary Care and/or Community Mental Health setting. Roles are open to all professions.
You will be a confident assertive communicator with colleagues and stakeholders alike.
Above all you will have the ambition and drive needed to work within an environment of change and innovation.
There will be a requirement to travel between various GP surgeries in the locality, in a timely manner.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
