Job summary

Band 6 Community Mental Health Practitioner

The post holder will be a registered RMN or Occupational Therapist and will be an experienced practitioner within the multi disciplinary team.

The post holder will work as a clinician in the Community Mental Health Teams, with those aged 18 yrs + with a particular focus on those of all ages experiencing memory problems, and those experiencing functional disorders such as depression, anxiety, psychosis

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

  • Assessing, treating and supporting those with mental health conditions/suspected MH conditions in the community
  • Supporting service users to function to their optimal ability in their environment. This can involve working in a diverse range of settings with a range of associated demands. For example: challenging behaviours and/or exposure to poor hygiene (personal/environmental).

o Maintaining and promoting a safe environment taking account of infection control and identifying and reporting hazards and risks in a timely manner.

o Accompanying, escorting and transporting service users as required.

o Working contracted hours as per the requirements of the service. May also be required, at times, to work at other Trust units and/or sites.

o Physically able to carry out all duties required of the clinical setting, including successful completion of physical interventions training as required.

Lone working within the community - access to individuals' homes

Considering safeguarding needs whilst working with adults experiencing mental health difficulties and for whom the potential for vulnerability is high
