Community Mental Health Practitioner

Job summary

We are seeking a Community Mental Health Practitioner who is a key clinical leader within the multidisciplinary care team and will have continuing responsibility to assess, plan, coordinate, deliver and evaluate care, communicate findings, influence and drive change and promote health and best practice, patient safety and experience.

We are looking for a qualified Band 6 Social worker, Occupational therapist or mental health nurse.

The Community Mental Health Practitioner will be professionally accountable to the Executive Director of Nursing and operationallyaccountable to the Service Manager/Matron.

Job holders must be able to work competently in a challenging environment and be able to react to changing care demands whilst demonstrating the highest levels of care and empathy towards patients.

This is across all aspects of the role in a flexible manner, demonstrating the trust values.

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

o Carry out and coordinate comprehensive, systematic assessments which take account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with service user and others through interaction, observation and measuremento Work collaboratively with other disciplines, service user and their carers to agree a holistic person-centred care plan that addresses the needs identified through assessmento To deliver and evaluate safe, person centred care in partnership with the service user and their carers to support recoveryo To act as a clinical leader supporting best practice in line with NICE guidanceo To facilitate and review approved Quality Improvement initiatives within the care setting.

Training Qualifications & Registration

Essential

  • RMN/RNLD with NMC registration, or OT with Registration with the Health and Care Professions Council (HCPC), completed preceptorship and practice assessor training.

Desirable

  • Significant experience at Band 5. Evidence of post qualifying continued learning.

Knowledge & Skills

Essential

  • To have sound knowledge of the Mental Health Act, and ability to apply relevant legislative frameworks and policies relating to care in the community..

Desirable

  • Ability to support and develop an environment which ensures effective clinical care and innovation
