Job summary
The Community Rehabilitation Service provides a flexible range of mental health interventions to help clients complex mental health needs to manage their mental health more effectively and remain in their own homes.
The team works in close partnership with Social Care and housing providers to ensure each client has the necessary resources available to enable them to engage in their rehab journey. Across the Community Rehabilitation teams, the clinical team consists of Occupational Therapists, Psychologists, Nurses, and VCSE Workers.
We currently have an exciting opportunities for two enthusiastic and motivated Healthcare Support Worker to join our West Kent Community Rehabilitation Service. Post one will cover Maidstone (base will be Albion Place). Post two will cover South West Kent (base will be Highlands House).
Individuals may be expected to ensure the team is able provide input between the hours of 08:00 and 20:00 according to the needs of the service, including working some evenings, weekends and public holidays.
Main duties of the job
- You will work as part of a Community Rehabilitation Team , focusing on the direct needs of service users.
- Under the guidance and supervision of the Multi-Disciplinary Team, you will provide support and give time to an allocated group of service users in order to promote their recovery and maintain them in their community environment. You will be promoting and facilitating therapeutic activity, promoting self-care and supporting with activities of daily living.
- You will support service users to identify their needs and to assist Clinicians to plan, implement and evaluate individual care plans.
- You will be working to the recovery model to support the service user to work towards their identified goals.
- Giving physical health checks and physical health care as required or instructed, documenting the physical health checks on the early warning scoring system (MEWS), chart and reporting changes to the nurse/healthcare professional in charge.
- Undertaking day-to-day tasks as allocated by Nurse/healthcare professional.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
