Job summary
We have an exciting opportunity for a Band 5 Community Mental Health Nurse to join our Home Treatment Team, based at Priority House in Maidstone.
You will be working within the home treatment function delivering support to patients and their families using evidence based, profession specific and general mental health knowledge, providing client centred, recovery orientated principles and practice for individuals, families and carers referred to the Service.
Working in accordance with Home Treatment Team Policies and Procedures and the Care Programme Approach and providing an alternative to inpatient admission, treating patients in their home environment who are presenting in a crisis with significant distress.
Main duties of the job
Work within their professional code of conduct.
Work as part of the Multi-Disciplinary Team driving the implementation of the nursing care as per the service user's care plans, meeting the physical & emotional needs of the service users in a caring and empathic manner.
Take a leading role in monitoring & maintaining physical wellbeing.
Work flexibly regarding shift changes according to the needs of the service.
Affectively record and communicate information relating to service users.
Delegate duties and support the MDT along with unqualified staff, HCAs and students
Be aware of the trust values & behaviours and use them to provide a framework for your daily work.
Undertake any reasonable request within own competencies and understanding.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
