Job summary

Join our established Medway Crisis Resolution and Home Treatment (CRHT) Team and make a real difference in the lives of individuals experiencing acute mental health challenges.
As a CRHT Support Worker, you will play a vital role in our dedicated team, providing crucial support to individuals experiencing acute mental health episodes within the comfort of their own homes. You will contribute to delivering the highest level of care, enabling patients to remain in their familiar surroundings while receiving the support they need.
Key Responsibilities:
- Providing direct support to individuals experiencing acute mental health distress in the community.
- Assisting patients who have recently been discharged from inpatient mental health wards, facilitating their transition back home
- Working within our Rapid Response function, undertaking initial assessments alongside a qualified mental health professional, responding to referrals from the Urgent Mental Health Helpline (available 24/7)
This is a permanent role at the agenda for change band. The service covers a 24-hour shift rotation over 7 days a week.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job

- Working collaboratively with other members of the multidisciplinary CRHT team.
- You will need experience of working in a mental health care setting and have skills and experience in delivering a menu of interventions aimed at promoting recovery such as: health promotion, anxiety management, social inclusion, emotional first aid, self harm reduction and safety planning in collaboration with patients, carers and social support networks.
