Job summary
To work as a member of the Crisis Resolution Home Treatment Team, working within the home treatment function delivering support to patients and their families using evidence based, profession specific and general mental health knowledge providing client centred, recovery orientatedprinciples and practice for individuals, families and carers referred to the Service. You will also work within the Rapid response function undertaking assessments 24/7 received from Urgent Mental Health Helpline as well as undertaking gatekeeping assessments. To work in accordance with Trustand Crisis Resolution Home Treatment Team Policies and Procedures and the Care Programme Approach. To provide an alternative to inpatient admission treating patients in their home environment who are presenting in a crisis with significant distress.
Main duties of the job
o To work collaboratively with MDT members to contribute to a flexible and effective interdisciplinary team approach to clinical care.o Communicate face to face with clients on a daily basis, some of which display challenging behaviour.o To effectively establish a therapeutic relationship with service users and their relatives/carers.o To assess capacity and obtain informed consent from service users and work within a legal framework with those clients who lack capacity to consent to treatment.o In conjunction with colleagues to undertake, record, review and work within individual risk assessment and management plans in line with service and Trust policy.o To provide mental health assessment of clients with acute mental health needs, clearly assessing and managing risks.o To lead joint reviews for patients with complex needs to plan transfer of care.o To ensure service users are involved in the planning and prioritisation of their care plans wherever possible.o To adapt practice to meet individual service users' circumstances, including regard for cultural and linguistic differences.o To work collaboratively with service users, staff, and relatives to identify goals and determine and deliver an appropriate intervention plan which aims to maximise engagement and performance whilst balancing risk, and promoting choice and independence.
