We are looking for individuals who will:

Operate as a Specialist Nurse for CPA level clients (and where appropriate non-CPA level clients) with complex and diverse needs.

Undertake comprehensive assessments, developing and reviewing care-plans.

Manage and ensure the provision of appropriate medical and psychosocial interventions to a caseload of service users with learning disabilities who experience mental health problems. To effectively meet the needs of the client group and their careers in line with agreed NHS and Trust competencies and targets. To ensure that the care offered is evidence based and in line with NICE guidelines.

Ensure information systems and client records are effectively maintained in accordance with the Trusts policy. To input data concerning clinical activity as required ensuring that entries are accurate and timely. To ensure that procedures are followed in order for necessary data to be collected (e.g.) key performance indicators.

Ensure that the views of the service users and carers are sought and incorporated into treatment plans and that care is sensitive to the needs of all our service users.

Participate in crisis intervention and provide clinical cover for team members in their absence.

Liaise with crisis and inpatient services as required. And, to provide written reports, and CPA Planning Meetings, where appropriate, ensuring that key professional input and expertise is provided.

Ensure effective verbal and written communication with all staff, service users and carers in order to maximise user and carer involvement in the delivery of care and treatment. Liaise as required with other disciplines and organisations to ensure that all issues affecting service users care is managed and communicated to a high standard

Participate in Mental Health Act Assessments of service users that you care coordinate. To be responsible for the administration of and or monitoring of medication, ensuring that all service users receive adequate information about treatment and any possible adverse effects. Participate in research / audit / clinical governance as negotiated.

Actively participate in and contribute to team meetings. To undertake other duties as appropriate to grade, as requested by the Team Leader/ Senior Practitioner. To participate in the mentoring and supervision of students.

Where negotiated, attend training courses as required and necessary in order to compile a portfolio of evidence, supported by experiences in the field to achieve appropriate competency levels within practice.

Maintain an awareness of legislation, directorate and corporate policies and procedures, particularly those relating to, Winterbourne Review, CPA and Care Coordination in order to ensure consistency and a high quality of service delivery across the Directorate

Develop partnerships and participate in working with TPLDs, Acute Services, and Private Sector, Voluntary Agencies, and carers in order to facilitate a safe and effective service delivery.

Abide by the Code of Conduct pertaining to your own profession.

Abide by the policies and protocols of the Kent & Medway NHS and Social Care Partnership Trust.

Keep abreast of all developments pertaining to the provision of mental health of learning disability and contribute to and review the development of ongoing initiatives for service development within the specialist directorate.

Key Objectives

The postholder will be required to:

Promote the well-being and mental health of service users with learning disabilities, by providing relevant information and advice to them, their families and to supporting agencies.

Participate in the process of the Care Programme Approach, attending clinical review meetings and multi-agency/CPA meetings, as appropriate.

Actively participate in the evaluation and development of the service by attending team and service improvement meetings.

Work in collaboration with all members of the multi-disciplinary team, and the wider trust including voluntary and other agencies.

Maintain a clinical caseload, planning and prioritising workload.

Develop and provide evidence-based treatment plans for identified service users in conjunction with the patient, family and referrer.

Formulate, implement and evaluate treatment programmes.

Communicate effectively within the MHLD Multi-disciplinary Team, CMHT, Inpatient Services and Crisis Resolution and Home Treatment Team as well as colleagues from other directorates of Kent and Medway NHS and Social Care Trust.

Communicate effectively with external voluntary and statutory agencies, including CLDT.

Maintain accurate and relevant records and reports, in accordance with Trust policy and professional requirements.

Share information with appropriate services in line with CPA Practice and Caldicott guidelines.

Liaise with other professionals and agencies that may be involved with a particular case.

Demonstrate a commitment to his or her own, professional development through accessing appropriate training and development opportunities

Participate in Trust mandatory training programme.

Participate in clinical and managerial supervision.

Attend training and educational activities identified within personal development plan relevant to the speciality.

Undertake regular appraisals with their line manager and to identify own development needs, ensuring that skills, knowledge and competencies are continually updated and extended in line with lifelong learning.

Undertake any other duties that are commensurate to the grade of the job.

Registration

Essential

• Qualification - LD / MH Registered Nurse with current NMC Registration

Desirable

• Qualification - LD / MH Registered Nurse with current NMC Registration

LD / MH Registered Nurse with LD experience

Essential

• Experience working with LD is essential
• Experience of providing thorough baseline assessments and case formulation
• Good interpersonal skills and an ability to communicate effectively with service users, carers and colleagues
• A working knowledge of mental disorders, treatment options and approaches, and assessment frameworks A good knowledge and understanding of CPA and Care Coordination

Desirable

• Community Evidence of post registration training related to the provision of mental health of learning disabilities or other evidenced based therapies. Mentorship/ENB 998 Qualification or equivalent
• At least two year's post-qualification practice in Mental Health
• Experience of working in a community setting and the ability to undertake home visits/attend various clinical sites as required
• Computer literate
• Up-to-date knowledge of research of MHLD
