Job summary
We have an exciting new opportunity within South West Kent Community Mental Health team , as we look to develop a Senior Community Mental Health Nurse position providing guidance and support to the team at a senior nurse level. The team covers the South West Kent area for working age adults (18-65).
You will have continuous responsibility as lead health care professional for a complex caseload. You will liaise between stakeholders to ensure clear communication and seamless care pathways, thus enabling access to the appropriate level of care and intervention.
You will provide senior clinical leadership, providing advice on all nursing issues and will form part of the local leadership team, working collaboratively alongside the operational team leaders and senior clinicians.
You will take a lead role in the development of student nurses, newly qualified nurses and staff new to community working.
Main duties of the job
You will facilitate clinical sessions offering short-term interventions based on psychological therapies and offer professional advice to client/patients and their carers, other professionals, statutory and voluntary agencies. You will act as lead professional for patients with complex needs, providing holistic, autonomous nursing care and ensuring that each patient has a defined individual care programme to meet their needs.
You will maintain communication with other professionals and agencies and will facilitate the transfer of patients between primary and secondary care and between the different specialist teams within the Trust.
You will provide specialist mental health advice and education to staff within the team including clinical supervision. You will act as a role model for colleagues and others in relation to professional and personal conduct and practice and be a mentor and adviser for student nurses, newly qualified nurses and staff new to community working. You will work closely with the Student Placement Facilitator to ensure that the learning objectives for students are consistent with the requirements of their educational programmes.
