Job summary

The role gives opportunity for developmental and creative thinking in shaping an excellent social work model in a multi disciplinary team.

The work around the implementation of the Community Health Framework for mental health within the PCS will enable Social Work to develop a strong profile within this sector.

As a Senior Social Worker, you will provide a high standard of support and care delivery with regards to social work needs to the 3 older adult mental health wards in East of the County with patients / service users whose ability to cope with the tasks of living are threatened or impaired by mental health issues, emotional, psychological and social problems. You will work closely with patients, families, carers other agencies (internal and external) and individuals to support patients / service users with a clear focus on recovery, social inclusion and social justice. The role will entail assessment, support planning and the opportunity to provide 1-1 strength based therapeutic interventions/social work interventions and facilitate early discharge planning.

To carry out this role successfully you will have:o A recognised Social Work qualification.o Evidence of further post qualification social work training.o Experience of practicing in a multi-disciplinary team and with external resources.o Demonstrable experience of working in the field of Mental Health.

  • Demonstrable knowledge and experience of safeguarding adults/statutory procedures.
  • Excellent IT skills including experience of using a case management system.
  • Experience of implementing and adhering to quality assurance processes in support of service delivery.You must be prepared to travel in the course of your work and a full driving licence and use of car are essential.

  • Within this service Social workers will focus specifically upon the needs of older adults with functional and organic mental health issues.

  • Provide professional guidance, support and developmental opportunities with less experienced or knowledgeable social work colleagues, including taking the role of supervisor, practice educator with social work students/junior staff.
