Job summary

We are currently seeking to recruit a Band 6 Mental Health Practitioner into a 12 Month Fixed Term Contract within the Wakefield Early Intervention in Psychosis Service (Insight Team). The team covers the Wakefield locality, working with people aged 14 to 65 and stretches across adult and childrens services. The team forms part of the community service line and Enhanced Pathway within the Wakefield BDU. The post holder would be expected to work across the Wakefield area with a named base currently in the Drury Lane Health & Wellbeing Centre.

You need to be an individual who is self motivated, enthusiastic, and who has a desire to work to improve life chances of people who may be experiencing their first episode in psychosis. This can take a lot of patience, requiring keen skills in creative engagement. Part of the role will involve health promotion; improving the physical heath of this group of service users as well as promoting the role of the insight team to capture appropriate referrals

The right value base and attitude is just as important to us as experience. Working within an Early Intervention Team demands the ability to challenge traditional medical models and embrace a psychosocial approach to delivering care.

Main duties of the job

You will need to demonstrate your continuing professional development since the point of qualification and how this can transfer to the role. You need to be open to enhancing your current skills through in house training and the shadowing of team members. You must be willing to attend external training as required for the post. You will provide highly specialist skills and expertise, facilitating interventions of the highest quality in the field of mental health, including emergency assessments and intensive interventions with children, young people and their families. Work autonomously and collaboratively within the MDT, providing consultation and teaching for other agencies/professionals. Provide supervision and teaching to all other disciplines within Insight EIP. Promote safeguarding and the welfare of children and young people

Key Result Areas:

Management of own caseload in line with C.P.A. procedures and risk management protocols.

Provision of leadership to the Early Intervention in Psychosis team in respect of psycho-social interventions to people and their carers in line with national and local practice standards e.g. NICE

Supervision and support of other team members.

Participation in service development and clinical governance structures.

Professional:

Assesses needs and identifies problems relevant to the care of people referred.

Devises a plan of care in partnership with the person and carers.

Implements the planned programme of care to ensure a high standard is achieved.

Reviews the effectiveness of the care provided and where appropriate initiates any action.

Advises and supports the person and carers to promote health and well-being and to prevent illness.

Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required.

Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community.

Functions as a member of a multi-disciplinary team.

Management of enquiries and direct referrals of people in accordance with Early Intervention in Psychosis team operational policy and participates in the team duty rota.

Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

Responsible for acting up in the absence of the team leader

Administrative:

Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.

Participates in the formulation of policies at unit level.

Maintains accurate mileage records.

Manages caseload with due economy of cost and time.

Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only).

Responsible for the ordering of stock with due economy.

Produces monthly statistics in line with trust directives.

Keeps the Management Team and colleagues informed of developments relevant to the area of responsibility.
