Single Point of Access Older Peoples Service

My name is Toni, and Im Team Manager at the Single Point of Access for older people. We have 1 full time vacancy and 1 part time vacancy.

Our team is the main access route into Wakefields older peoples mental health services, including referrals for memory assessments. People referred to the service are assessed by the multi-disciplinary team to determine the most appropriate service and support required, which may include signposting to internal and or external services and agencies.

You will work alongside a supportive team, triaging and assessing older people who are concerned about memory and cognition. You will provide specialist assessment, advice and refer into memory clinic if appropriate.

We will offer you an induction thats tailored to your needs and experiences, including preceptorship.

Main duties of the job

You will be required to conduct initial assessments, completing necessary assessment documentation including risk assessment. You will participate within the MDT and be part of formulating treatment planning. On a rota basis, you will provide support and assist the designated duty worker triaging referrals and answering calls and queries coming into the team.

We currently work Monday to Friday 8am to 6pm, working across 2 shift patterns 8am-4pm and 10am-6pm.

As this is a community role and we work agile, you will need to demonstrate ability to travel across the Trust as per the role demands. Although there will also be the opportunity to offer assessment in clinic across the district.
