Job summary
We have an exciting opportunity for a full-time Speech & Language Therapist to join the Intensive Support Team (IST) in the Wakefield.
The post is being offered as either a Band 5- 6 development position or Band 6 for a suitably qualified clinician. The successful Band 5 candidate will be supported through the required Preceptorship period. Band 6 candidates who are not yet qualified Dysphagia assessors, if successful, would be offered the opportunity to complete the appropriate dysphagia course.
The primary function of IST is to provide intensive assessment, formulation, and intervention within a Positive Behavioral Support Model to complex service users with behaviors that challenge to reduce the risk of placement breakdown, hospital admission and/or contact with the Criminal Justice Service.
You will join a well-established multi-disciplinary team which includes Dietetics, Occupational Therapy, Physiotherapy, LD community Nurses, Psychiatry, Psychology, MDT Intensive Support Team, as well as Therapy Assistants & Healthcare Support Workers.

Main duties of the job
You will be a highly motivated and conscientious individual preferably with experience supporting people in a variety of activities and community settings.
You will work both independently and within an MDT; and this will involve assessments and reports to contribute towards Positive Behavioral Support plans/interventions, and liaison with families and support staff to implement communication interventions.
You will be responsible for care coordinating a small caseload, showing an understanding of the complex needs associated with the learning disability population.
You will also demonstrate qualities to deliver safe, effective and personalised care ensuring processes are followed in accordance with Trust values and objectives.
The role requires close working with the IST MDT clinicians where you will use your speech and language therapy skills (i.e. communication & dysphagia assessment to address the holistic needs of service users with a focus on PBS and care coordination.
It is essential that applicants can travel independently across the locality and have access to a suitable vehicle for business purposes.
