My Name is Sarah Sedgwick and I
am the Team Manager for the Wakefield Intensive Home Based Treatment Team. An exciting opportunity has arisen for a band
5 practitioner for the Intensive Home Based Treatment Team in Wakefield. This
is a multi-disciplinary team of Social Workers, Nurses, Medical Practitioners,
Psychologists, Trauma Informed Personality Disorder Practitioner, Dual
Diagnosis Lead and a Non Medical Prescriber, as well as Health Care Support
workers and Admin staff.
The team is the single referral
point for people in crisis, offering assessment and intensive home-based
treatment for those for whom hospital admission would otherwise be necessary. We
promote the patients well being through psycho-social interventions, with the
main areas being recovery and social inclusion focused.
The service operates 24 hours a
day, 365 days a year. Practitioners work
a shift system however as a band 5 in the team, there is currently no
expectation that you would work night shifts. It would be essential that you
are a car driver as we cover a large geographical area.
Main duties of the job
The successful practitioner
will work within the remit of the band 5 Intensive Home Based Treatment
Practitioner job description.
The main part of your role
will be carrying out home visits to patients within the community, supporting
them with their current needs, reviewing their care and risks. Care will be
delivered to a high standard through assessment and effective risk management,
there will be an element of positive risk taking within the role. You will be
expected to formulate plans to support patients at home on an individual basis
and on a team basis. Where it is deemed unsafe for the patient to remain at
home, you will be an integral part of the gatekeeping for ward admissions. A
key element of your role will be your ability to work both autonomously when
making decisions in the community, but also sharing awareness and knowledge of
the whole team caseload with the other members of the team.
There will be opportunity to
take on special interest roles within the team and develop your skills. Supervision is carried out both
as a team and individual management supervision where your developmental needs
and wants can be discussed.

- Assesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action.
- To be competent in all aspects of care and risk management as defined within the framework of the care programme approach.
- To work collaboratively with clients, carers and multiple other agencies to provide effective care to optimize and maintain clients mental health and overall functioning, ensuring needs are met.
- Management of enquiries and direct referrals of people in accordance with the Intensive Home Based Treatment Team. Operational Policy and participates in the team 24 hour duty rota. To participate in the team duty rota including on-call system (if applicable to the business continuity of the service).
- Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
- To be responsible for the safe custody, transportation and administration of medicines including I.M. preparations.(if appropriate to qualification)

- Provide peer supervision and support to other team members and to take personal responsibility and supervise junior staff in the maintenance of written and electronic client and Trust records including inputting data related to own work activity. To identify any areas of concern to a senior member of staff.
