The staff team at the Horizon Centre are excited to welcome applications for a part-time/up to full time Senior Clinical Practitioner on our Assessment and Treatment Unit in Wakefield.

The Horizon Centre is an 8 bedded Assessment and Treatment Unit that provides highly specialised assessment, formulation, and intervention for adults with a diagnosed learning disability, working in a person-centred way to meet each individuals needs. The service works within Positive Behavioural and Active Support frameworks and provides trauma-informed care focused on treatment and recovery, with the aim of supporting the person back into the community and their everyday lives.

You will be joining an experienced and supportive team of doctors, nurses, nurse associates, trainee nurse associates, psychologists, healthcare assistants and allied health professions who work together with the person at the centre.

You will be supported by the wider multi-disciplinary team to ensure we continue to maintain high-quality, person-centred services in line with our national and locally agreed targets.

Main duties of the job

As a Senior Clinical Practitioner, you will be responsible for supporting the Advanced Nurse Practitioner in deliver high quality care on the ward by acting as a positive role model and leading by example. Acting on behalf of and to deputise, as directed by the Advance Nurse Practitioner in their absence to provide quality evidence-based nursing advice, support, and care for service users within a multidisciplinary setting.

The role will entail some management responsibilities including supervising staff, managing the day to day running of shifts, creating rotas, sickness monitoring and producing high quality care plans, risk assessments and reports.

The post holder will be responsible for overseeing the assessment, planning, implementation, and evaluation of nursing care, and have a working knowledge of local, national policies and legislation which govern current service provision for people with a learning disability.

JOB SUMMARY

The post holder will assist the Advanced Nurse Practitioner in the provision of clinical leadership and to be a clinical role model to the 24-hour inpatient team. Acting on behalf of and to deputise, as directed by the Lead Nurse in their absence and provide quality evidence based nursing advice, support and care for service users with mental and physical health needs and to their carers within a multidisciplinary setting.

The post holder will be responsible for overseeing the assessment, planning, implementation and evaluation of nursing care within agreed parameters, and have a working knowledge of local, national policies and legislation which govern current service provision for people with a learning disability

The post holder will have a good knowledge of positive behavioural approaches and apply this in their own practise as well as supporting others in the ward team.

The post holder will be responsible for identifying and contributing to the training needs of junior

registered and unregistered staff, and ensure that issues of equality, diversity and rights are dealt

with in accordance with good practice and legislation.

They will ensure that effective communication pathways exist, and that they act within their sphere

of responsibility, managing the clinical area in the absence of the ward manager

Clinical

To be responsible for supervising the role of the named nurses in relation to care planning within the clinical area.

To be responsible for the assessment, planning, implementation and evaluation of the evidence based care required, including health promotion for a named service user/group ensuring it meets Trust timescales.

To participate in the setting of quality standards, including the monitoring and reviewing in line with current clinical guidance practice and policy. Utilising the data available such as learning from incidences and complaints.

To ensure the post holder understands and meets their professional role and responsibilities under the Care Programme Approach.

To ensure that the requirements of the Mental Health Act and Mental Capacity Act are observed, adhered to and implemented.

To be responsible for the promotion of carer and service user involvement within the service and for its provision.

To undertake risk management and risk planning..

To be responsible for establishing therapeutic relationships with service users, and the implementation of evidence based therapeutic interventions with appropriate boundaries in accordance with Nursing and Midwifery Councils professional code of conduct.
