Job summary

Hello, my name is Katy Miller, I am the team manager for the older adult care home liaison team (CHLT) in Kirklees.

We are seeking to recruit someone who matches our values and who can support us to continue to provide high standards of assessment, treatment and care to our residents in care homes.

This is an opportunity to work in a community team having a strong focus on liaison activity and working collaboratively with residents, families, colleagues, and care home staff to deliver timely assessment and interventions.

This is a dynamic and varied role where you will work as part of a team and autonomously.

In return you will be supported and encouraged to develop and grow as a clinical practitioner. There are multiple opportunities to develop your skills and knowledge with team support and supervision.

The team take a strong focus on staff well-being and actively seek ways in which we can look after each other.

This is a temporary/secondment opportunity to cover maternity leave for 12 months.

The role will be across Kirklees utilising our base at Beckside Court in Batley. Working Monday to Friday 9am-5pm. Flexible working options are available.

You will need to be a car driver and have the use of a vehicle to undertake any necessary travel in connection with the duties.

Main duties of the job

This role has been advertised previously, please do not reapply if you had applied within the past 6 months.

Involves independent caseload management, working autonomously whilst contributing to the functioning of a multi-disciplinary team.

Undertaking assessments of residents mental health and reviewing care plans.

Providing on-going education and advice to care staff to improve residents well-being and support management of risk.

Responding to duty calls, supporting colleagues, role modelling clinical decision making and prioritisation of workload in times of high clinical demand.

Working collaboratively with residents, families, care home staff; monitoring changes to psychiatric treatment regimes and promoting non-pharmacological interventions.

Documenting care delivery to a high standard in line with professional standards, local and national guidance.

Recognising situations that may be detrimental to the health and well-being of residents and initiating actions that may be required.

Supporting student education and contributing to the preceptorship and education of newly qualified staff where appropriate.

Maintaining professional registration and demonstrating commitment to statutory codes of practice and continued professional development requirements.

Management of enquiries and direct referrals of people in accordance with the Core C.M.H.T. Operational Policy and participates in the team duty rota. To participate in the team duty rota.

Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

To be responsible for the safe custody, transportation and administration of medicines including I.M. preparations.(if appropriate to qualification)

Education

Provide peer supervision and support to other team members and to take personal responsibility and supervise junior staff in the maintenance of written and electronic client and Trust records including inputting data related to own work activity. To identify any areas of concern to a senior member of staff.

To have an understanding of and participate in the clinical audit process.

To have an awareness of evidence based practice relevant to the specialist area and participate in the education of individuals and target group.
