Job summary
Join a small, supportive and
well-established team delivering a highly valued service across Kirklees. You
will work with children and young people (CYP) with learning disabilities in
special schools, providing direct care, advice, signposting, and training,
contributing to the SEND agenda.
The service receives a high
number of referrals, offering a varied and rewarding role where you will
complete assessments, deliver interventions, and manage your own caseload. You
will be supported through regular line management, caseload, and clinical supervision.
You will play a key role in
maintaining safe, effective care, managing demand, and supporting KPI delivery
within a collaborative and caring team environment.

Main duties of the job
Deliver high-quality, safe, and effective
care to children and young people, supporting positive patient journeys and
experiences. Undertake timely assessments and interventions, contributing to
the reduction of waiting times and ensuring equitable access to services.
Manage and prioritise a defined caseload, maintaining quality and frequency of
appointments in line with service standards.
Contribute to group interventions where
appropriate and support service delivery to minimise delays. Work
collaboratively with colleagues and partner agencies to maintain continuity of
care.
Promote staff wellbeing by working as part of
a supportive team, engaging in supervision, and contributing to a positive
working environment.
Support
delivery of service KPIs, including RTT and patient feedback measures, and
contribute to service improvement to meet commissioner expectations and
maintain high standards of care.

levels of ability as defined by the team criteria. The post holder will serve as an effective

shortcomings of the nursing service suggesting appropriate actions.
To ensure that the policy for safe usage and storage of
drugs is adhered to on Trust premises or in the community.
Provide clinical supervision and professional
advice/support to all nursing staff and other appropriate disciplines.
