We are excited to be
recruiting a Mental health nurse, Occupational Therapist or Social worker into
a community mental health practitioner post to join the Calderdale Early
Intervention in Psychosis service on a part time basis.
The Calderdale Early
Intervention in psychosis service is a well established MDT team working with a
wide age range of service users from the ages of 14 up to 65 promoting that
recovery is possible from first experience/episode of psychosis. The service in
addition offers care and support for those who are at risk of developing
psychosis.
The team is made up of Mental
health nurses, Occupational Therapists, social workers, IPS worker, Vocational
support worker, Support Workers, Peer support worker, and Consultant
Psychiatrist.
The appointed community mental
health practitioner would have the opportunity to offer care coordination
working in partnership with their service users and their families.
Interventions offered are to be based on NICE best practice guidelines for
Early Intervention in psychosis services, using a team approach which can be creative
and meaningful to service users and extremely rewarding.

Main duties of the job
- To offer care coordination for service users under CPA during their recovery from First episode psychosis
- Hold a caseload 15 EIP service users, ensuring that there mental and health needs are addressed through offering EIP Nice concordant interventions.
- Supporting service users and carers on their recovery journey, ensuring that risk is well managed and their care is well co ordinated.
- Care plans, Firm risk assessments, comprehensive assessments would be completed for all service users by community mental health practitioner, being understaffed may impact on the quality of the governance due to systems pressure.
- Use their professional skills to compliment the MDT working in a team approach model across the entire service caseload.
- Community mental health practitioners work in collaboration with the service user and their families to work towards achieving desired goals offering appropriate care and support within the service pathway timeframe
- Caseload would be a mixture of first episode of psychosis, At risk mental state of developing or those requiring further assessment

Management of enquiries and direct referrals of people in accordance with Early Intervention in Psychosis team operational policy and participates in the team duty rota.
Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
Responsible for acting up in the absence of the team leader
Administrative:
1. Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
2. Participates in the formulation of policies at unit level.
3. Maintains accurate mileage records.
4. Manages caseload with due economy of cost and time.
5. Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only).
6. Responsible for the ordering of stock with due economy.
7. Produces monthly statistics in line with trust directives.
8. Keeps the Management Team and colleagues informed of developments relevant to the area of responsibility.
