Community Mental Health Practitioner, In-reach Role

Job summary

The Home Treatment Team (Kirklees OPS) is looking to recruit an enthusiastic and autonomous Band 6 practitioner on a full-time basis. This is to build and improve on the already successful role of In-reach Liaison Worker.

The Home Treatment Team (Kirklees OPS) is a community based mental health team that provides crisis interventions and home-based treatment for older people whose mental health needs increase significantly, either due to an acute episode or relapse of an enduring mental health condition.

The In-reach Liaison Worker is a dedicated role within the Home Treatment Team (Kirklees OPS), working alongside both inpatient and community staff, to support timely discharge and provide a joined up and seamless journey for service users between the ward and the community.

Operating with a high level of autonomy you will be working with Kirklees service users who are admitted to the older adult mental health ward in Dewsbury, and on occasion, if a Kirklees resident is admitted to one of our other older adult wards in Wakefield, Barnsley and Halifax, to support timely repatriation to our Kirklees ward.

Main duties of the job

Your duties will include:

Working with service users admitted to the ward on a 1:1 basis, assessing need and risk, and supporting and assessing them during periods of home leave

Actively identifying inpatients who would benefit from community input from Home Treatment Team (Kirklees OPS) in order to facilitate timely discharge.

Providing support to service users families and carers, and working alongside them to identify any hurdles to successful discharge.

Establishing and maintaining positive working relationships between Home Treatment Team (Kirklees OPS) and ward teams, and acting as a point of contact between the two.

Participating in bed management meetings, inpatient ward rounds and Home Treatment Team (Kirklees OPS) MDTs.

The successful candidate will have excellent verbal and written communication skills, be able to establish positive working relationships across multiple services andbe able to autonomously manage and prioritise their time. They will also have well-honed and effective risk assessment and management skills and a compassionate and positive attitude towards the client group.

Full UK driving licence and use of own car is essential.

5. Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only).

6. Responsible for the ordering of stock with due economy.

7. Produces monthly statistics in line with trust directives.

8. Keeps the Management Team and colleagues informed of developments relevant to the area of responsibility.

Education:

Supervises students and trainees on placement.

Assists in the teaching and in-service training of staff.
