An
exciting opportunity has arisen for a Physiotherapist to join the Kirklees Community
Learning Disabilities Health Team. This is a full-time post and would be suited
for a newly qualified or experienced physiotherapist wanting a new challenge.
Our
well-established Team covers the whole Kirklees area and consists of a
comprehensive multi-disciplinary team including psychiatrists, clinical
psychologists, learning disability nurses, occupational therapists,
physiotherapist, speech & language therapists, dietician and healthcare
support staff. The Team works together deliver therapeutic assessment and
treatment and to support adults with LD and their families/carers.
The Trust is
committed to staff development and in-house training to enable the applicant to
fulfil the requirements of the role. There is a trust wide Physio Forum led by
the professional lead which offers support and CPD opportunities.

Main duties of the job
You will be
a highly motivated and conscientious individual, who has the desire to work
creatively with people with learning disabilities within the community.
We are looking for a dynamic person who will support the team to provide
the highest standards of safe and compassionate planned care to clients to
maximise their potential.
You
will be required to use your specialist physiotherapist skills to support the
assessment and delivery of high-quality therapeutic interventions to address
service users needs. This will require effective MDT working skills, close
liaison with other agencies and use of excellent communication skills.
The successful applicant will manage a
community caseload of service users who have a variety of presentations
enabling activity participation and independence in a community setting,
including training of family carers and paid carers in different settings. You would be required to evaluate
treatment outcomes and contribute to service development.
You will be working as part of a physiotherapy team within
the wider MDT and will receive support and regular clinical supervision.

JOB SUMMARY
The post holder will be
a core member of the physiotherapy service providing specialist therapeutic
interventions to adults and older people with mental health problems.
To assist in the clinical
activities of the therapy team and wider mental health services and
provide supervision to support staff.
To contribute to service
development and in the wider debate of physiotherapy issues, strategic planning and professional
development.
To utilise specialist physiotherapy
knowledge and skills to assess, formulate goals, implement treatment and
evaluate outcomes involving the service user and family/carer.
To independently manage a defined
caseload of people with a wide range of specific difficulties enabling the
person to fulfil their functional potential with supervision from a senior
member of staff when necessary.
To work as an autonomous
practitioner within a wider multi-agency community.
KEY RESULT AREAS:
1.Clinical
To
ensure therapeutic interventions are of a high professional standard, informed
by evidence based practice and involves the service user and carer in all
aspects of the care programme.
To
demonstrate knowledge and understanding of the effect of disability providing
training and advice on lifestyle changes/adaptations to the clients social and
physical environments.
To
undertake specific standardised and non standardised assessments to formulate
the overall treatment plan in conjunction with the service user.
To
interpret the assessment results in order to plan and implement a client
centred treatment programme on an individual and/or group basis using graded
activity to achieve therapeutic goals.
To
contribute to a co-ordinated team approach throughout the assessment and in
meeting the service users needs including developing and maintaining close
working relationships with all relevant services and agencies.
To
participate in the evaluation of the effectiveness of clinical interventions.
To
continue clinical interventions where necessary to assist in the transition of
skills from an inpatient setting to living independently within the community.
To
work effectively within the multi-agency team ensuring that both written and
verbal communication is clearly maintained to ensure a seamless service.
To
be responsible for the maintenance of appropriate clinical records concerned
with service user treatment, in accordance with risk management documentation
guidelines.
To
attend appropriate clinical team meetings, referral meetings, case conferences,
care programme approach meetings and any other as required by clinical need and
as directed by the Line/Service Manager.
To
provide accurate and timely reports, statistical data through the electronic
Clinical Information System and other information as required.
To
participate in monthly clinical supervision and to provide clinical supervision
to other therapy staff and Support Workers.
