applications for an experienced band 3 Health Care Support Worker to join the Calderdale
Learning Disabilities Nursing Team. Theteam is a multi-disciplinary team that provides specialist
health support to both individuals, and their wider support networks. The
administrative base is in the vibrant town of Hebden Bridge.
The team is multi-disciplinary and provides specialist
health support to both individuals, and their wider support networks. The team
works agilely and covers the Calderdale locality. The work is varied and will
give the right candidate the chance to develop a range of skills working
alongside team colleagues in IST, occupational therapy, speech & language
therapy, physiotherapy, psychology, psychiatry and admin.
An interest and
or/experience in working creatively with people with a learning disability
and/or Autism Spectrum would be an advantage.
You will need to be a car driver and have the use of a vehicle to
undertake any necessary travel in connection with the duties given the
geographical area.

Main duties of the job
We
are seeking to recruit someone who matches our values and who can support us to
continue to provide high standards of care and support, The
post is a key role within the LD Community nursing team and the wider
Calderdale LD health team.
This is an
opportunity to work in a community team supporting the nurses and wider team to
deliver evidence based clinical nursing support by supporting the assessment
and information gathering and implementation of care plans.
Clinical
nursing team support aims to assess and meet the varying needs of individuals
with a learning disability. This results in improving health status and
reducing the health inequalities. The postholder also holds a key role in
supporting health promotion to ensure a health preventative approach where
possible.
We welcome
creative practice, you will expected to work autonomously, with the appropriate
support and guidance.
Work collaboratively with service users and their carers as well as other
professionals.
Developing
meaningful therapeutic relationships ensuring care is positive and safety is
maintained.
Visa sponsorship under the Health and Care Worker visa route can
only be considered if the prorated salary meets the minimum threshold of
£25,000, as required by the UK Visas and Immigration. For Band 3 roles, only
full-time positions at the top of the pay scale meet this threshold. Therefore,
sponsorship may not be possible depending on salary increment placement and
working hours.
