Hello, my name is Katy Miller, I am the team manager for the
older adult care home liaison team (CHLT) in Kirklees.
We are seeking to recruit someone who matches our values
and who can support us to continue to provide high standards of assessment, treatment
and care to our residents in care homes.
This is an opportunity to work in a community team having a
strong focus on liaison activity and working collaboratively with residents,
families, colleagues and care home staff to deliver timely activity and interventions.
This is a dynamic and varied role where you will work as
part of a team and autonomously.
In return you will be supported and encouraged to develop
and grow as a support worker. There are multiple opportunities to develop your skills
and knowledge with team support and supervision.
The team take a strong focus on staff well-being and
actively seek ways in which we can look after each other.
This is a 12-month secondment opportunity to cover
maternity leave. The role will be across Kirklees utilising our base at
Beckside Court in Batley. Working hours are Monday to Friday 9.00 am 5.00 pm.
Flexible working options are available.
You will need to be a car driver and have the use of a
vehicle to undertake any necessary travel in connection with the duties.
This role does not meet the eligibility criteria for skilled worker visa sponsorship under the updated UK government regulations introduced following the Immigration White Paper on 12th May 2025.
Main duties of the job
A key element of the role is visiting residents in care home settings to follow up on care planning implemented by lead professionals in the team. This involves spending time with the resident, observing wellbeing, gathering updates/information from care staff & feeding back to lead professionals.
Developing meaningful therapeutic relationships ensuring care is positive and safety is maintained.
Recognising situations that may be detrimental to the health and well-being of residents and seeking support to initiate actions that may be required.
Working autonomously & effectively managing your time whilst supporting colleagues & prioritising workload in times of high clinical demand.
Contributing to the functioning of a multi-disciplinary team.
Work collaboratively with residents, care home staff; offering education, advice and support (within scope of job role) to promote health and well-being.
Document care delivery to a high standard in line with professional standards, local and national guidance.
Support student education by providing shadowing opportunities to enable them to understand the role of a HCSW.

- Apprenticeship level 3 or equivalent eg, RQF(regulated Qualification Framework)NVQ level 3 in Health & Social Care or equivalent.
