Job summary

37.5 Hours Per Week

As an Advanced Practitioner you will work as a member of the multidisciplinary team within the North Kirklees enhanced team providing specialist assessment and treatment interventions for individuals with a diagnosis of a personality disorder, or those presenting with similar difficulties.

You will provide clinical leadership within the team in regards to this client group and work alongside the Team Manager to embed the pathway within the team. You will also work alongside the pathway leads to further develop good practice and ensure robust governance structures are in place and adhered to.

You will be required to attend managed clinical network meetings and actively support the structures in place which assist in providing excellent trauma informed care to this service user group.

Your role will involve liaising with service users, family and carers in addition to third sector, primary care colleagues and other external services so excellent communication skills are essential.

Main duties of the job

Managea small complex caseload of patients whilst holding the recovery model at the centre of your work. You should hold a formal qualification in Dialectical Behavioural Therapy (DBT).

You should contribute to the on-going development of the Trauma Informed Personality Disorder Pathway whilst working within the MDT.

You should contribute to the leadership within the North Kirklees Enhanced pathway, and provide advice and support for the staff team.

The post is a specialist role requiring an advanced level of knowledge and application, which will hold the Recovery Model at its centre and will work closely with other parts of Mental Health Services (Crisis Team, Inpatient services etc) and partner agencies to ensure effective assessments, care plans and monitoring of service users with a Personality Disorder.

The post holder will manage a small complex caseload and will joint work alongside the wider MDT.

The post holder will hold a formal qualification in Dialetical Behavioural Therapy and will form part of the wider locality DBT Managed Clinical Network.

The post holder will provide expert clinical leadership within the locality and will provide supervision and training on Trauma and Personality Disorder.

The post holder will assist and contribute to the on-going development of the Trauma Informed Personality Disorder pathway and ensure good communications between Mental Health Services and partners.

Ability to: Involve service users in all aspects of the care they receive and provide the highest quality of clinical care.

This will be achieved by:

Ensuring a timely acceptance and assessment of Service Users referred to the Trauma Informed Personality Disorder pathway

Providing service users with written information about services.

Ensuring that there is evidence that service users are involved in planning of and satisfied with their own care.

Working collaboratively with multidisciplinary colleagues and other agencies.

Carrying out full mental health assessment with service users utilising structured and evidence based assessment and outcome measures to enhance clinical effectiveness and complement role.

Formulating, implementing and evaluating interventions with individuals which are user focused and recovery based using a psycho social approach.

Embracing diagnostic uncertainty and assess/formulate and plan appropriate short term strategies as part of directing inappropriate referrals to the most appropriate service.

Providing specialist support and advice to professionals in relation to practice development.

Carrying out robust risk assessment in accordance with Trust/Organisational protocols.

Taking on the Care Co-ordination role of service users with complex needs working alongside other team members.

Engaging with families and carers.
