An exciting opportunity has arisen for a
highly motivated, passionate specialist community learning disabilities nurse
to join the Calderdale Adult Community Learning Disabilities Health Service.
We are
based in the heart of Hebden Bridge, a vibrant town on the edge of the
picturesque countryside. We benefit from free parking in the town centre. We
offer an agile working policy with opportunities for home working as well as
alternative bases to support effective cover of the wider geographical area of
Calderdale.
You will join an experienced,
well-established, and supportive team who work agilely across Calderdale. The team
consists of a multi-disciplinary team including psychiatry, psychology,
learning disability nursing, occupational therapy, physiotherapy, speech &
language therapy, dietician, therapy assistants and healthcare support workers.

Main duties of the job
You will be responsible for a
defined caseload with service users of all levels of ability.
You will serve as an effective
member of the MDT promoting a consistent multi-agency approach including
support, supervision and development of staff.
You will work flexibly based
on service user need, using reasonable adjustments to provide support.
You will demonstrate an
ability to work autonomously and as part of a wider team.
You will have an understanding,
of the complex needs of people with learning disabilities and demonstrate
qualities to deliver safe, effective and personalised care ensuring processes
are followed in accordance with Trust objectives.
You will be committed to team
working, a be highly resilient and self-motivating, determined to add value to

Due to the nature of the role,
it is essential that applicants are able to travel independently across the
locality and have access to a suitable vehicle for business purposes.
