Hello, my name is Judith Megson, I am the team manager for the
older adult community mental health team (CMHT) in Kirklees.
We are seeking to recruit someone who matches our values
and who can support us to continue to provide high standards of assessment,
treatment and care to our service users, to enable them to live well in their
community.
In return you will be supported and encouraged to develop
your skills and knowledge so that you continue to develop and grow as a
clinical practitioner.

We are looking to recruit an enthusiastic and motivated
qualified practitioner who can work in a flexible, innovative and creative way.
The role will primarily be based in Huddersfield, however, as the team has 2
geographical bases, you will be required to deliver a mental health service
Kirklees wide in line with clinical demand.
The teams core working hours are Monday-Friday, 9am-5pm.
Flexible working options are available.
You will need to be a car driver and have the use of a
vehicle to undertake any necessary travel in connection with the duties.

Main duties of the job
A key element
of the role will be care coordination, supporting colleagues, duty worker and
prioritisation of workload in times of high clinical demand.
Undertake
assessments and review care plans of service users which promote engagement,
stability, recovery and risk management.
Manage a varied
caseload covering functional and organic mental health presentations.
Work
autonomously but contribute to the functioning of a multi-disciplinary team.
Work with
service users with both short-term interventions and severe and enduring mental
health needs to promote health and well-being.
Document care
delivery to a high standard in line with professional standards, local and
national guidance.
Recognises
situations that may be detrimental to the health and well-being of the service
user and initiates action that may be required.
Support
student education when allocated to the team and to contribute to the
preceptorship and education of newly qualified staff where appropriate.
Administration
of depot medication in the community.
Maintain
professional registration and demonstrate commitment to statutory codes of
practice and continued professional development requirements.
