Job summary

An exciting opportunity has arisen for a B5 Community Mental Health Practitioner to join the South Kirklees Core Team.

The wider multidisciplinary Core Team includes CPNs, Social Workers, Psychological Therapists, Psychiatrists, Support Staff and OTs working with working age adults experiencing a range of moderate mental health needs. The team offers a range of therapeutic interventions on a short-term basis, generally for up to one year. Interventions provided can include support focusing on crisis and contingency planning, distress tolerance, emotional regulation, psychological therapies or support around diagnosis and medication issues.

You would work to enable people to overcome barriers caused by an impact upon their mental health due to social, economic, physical and / or psychological factors, or whatever is preventing them from living life how they would like to. You would deliver a high quality, co produced service with a range of service users on your caseload.

Main duties of the job

The role of the B5 MHT is to deliver crucial therapeutic interventions whilst also assessing and reviewing progress and potential barriers to recovery. The role will require working in service users homes and co producing care plans which identify specific interventions as well as 3rd sector organisations that may be more appropriate for them. Managing your own time and working with a high level of autonomy in the community will be important. There will be opportunities to access a range of development and training, as well as clinical supervision provided by senior staff in Core and opportunities to access group forums to reflect on practice.

This is also a challenging role, so youll need to be resilient, empathetic, and dedicated to delivering quality evidence-based interventions, sometimes with service users who display high risks to self or others. As a people person, with your can do attitude and strengths-based approach, youll build a good rapport with your service users, gaining a real understanding of their individual needs and personal barriers to recovery.

This role would suit an individual who has experience of supporting service users within health, social care or the voluntary sector, working specifically with people who have experienced serious mental illness.

  1. Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

Provide peer supervision and support to other team members and to take personal responsibility and supervise junior staff in the maintenance of written and electronic client and Trust records including inputting data related to own work activity.   1. To have an awareness of evidence based practice relevant to the specialist area and participate in the education of individuals and target group.
