An exciting opportunity has arisen for a Senior community learning disabilities nurse to join Barnsley Adult Community Learning Disabilities Health Service.

You will join an experienced, well-established and committed specialist multi-disciplinary team who work agilely across a number of locations in Barnsley and the surrounding areas. The Team consists of a comprehensive multi-disciplinary team including psychiatrists, clinical psychologists, learning disability nurses, occupational therapists, physiotherapist, speech & language therapists, dietician, therapy assistants and healthcare support workers. The Team works together to support individuals age 18 and over with diagnosed learning disabilities and their families/carers.

Main duties of the job

You will carry the responsibility for a defined caseload this will include service users of all levels of ability.

You will serve as an effective member of the MDT and promote a consistent multi-agency approach including support, supervision and development of junior staff.

You must be able to work flexibly based on service user need and using reasonable adjustments to provide support to service users, family and carers.

You will need to demonstrate an ability to work autonomously and as part of a wider team.

You will have an understanding of the complex needs of people with learning disabilities and be able to demonstrate that they have the qualities to deliver safe, effective and personalised care to meet the needs of the service user ensuring systems and processes are followed in accordance with Trust objectives.

You will be committed to team working, a highly resilient and self-motivated individual who is determined to add value to the organisation and is committed to our vision and values.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes.

The post holder will contribute to the leadership and development of the Specialist Learning Disability Health team in accordance within local and national guidance

The post holder will have the ability to work independently as specialist individual practitioner and make autonomous decisions as required. Have good clinical, management and leadership skills.

KEY RESULT AREAS:

Management

To promote positive leadership through excellent communication and interpersonal skills.

To liaise with members of the multi-agency, multi-disciplinary teams.

Support the effective management of resources and delegation as required.

Provide audit information as necessary.

Manage patient crisis and respond in a timely manner.

Contribute towards care management processes where appropriate.

Manage an individual caseload by prioritising need.

Support junior nursing staff in the implementation of their duties.

To be aware of current legislation, terms and development within Learning Disability Services.

To undertake training and continued professional development as agreed with the lead nurse/operational team manager

To advise the Lead Nurse/Operational Team Manager of shortcomings of the nursing service suggesting appropriate actions.

To ensure that the policy for safe usage and storage of drugs is adhered to on Trust premises or in the community.

Provide clinical supervision and professional advice/support to all nursing staff and other appropriate disciplines.

Liaise with specialist clinics in relation to medical and patient nurse support where appropriate

Support health improvement initiatives.

Ensure that professional registration is maintained.

Personnel

Maintain staff moral and provide junior staff emotional and professional support and supervision.
