We are seeking a dynamic, motivated, & enthusiastic Therapy Liaison to join our forward-thinking Neighbourhood Rehabilitation Service (NRS), working at the heart of our multi-disciplinary team
You'll play a key role in enhancing each patients journey through our service. Working alongside a skilled group of Allied Health Professionals, you'll help to co-ordinate the rehabilitation pathway for individuals recovering from illness or injury
Your work will involve:
- Supporting patients, families & carers to ensure a smooth, well-informed discharge process
- Co-ordinating practical support, including social care arrangements & community-based services
- Connecting patients with local agencies & ongoing health resources
- Helping our clinical team manage a varied & rewarding caseload

Main duties of the job
As Therapy Liaison you will
- Be part of a passionate & collaborative team
Liaise with patients, family & members of the MDT on a regular basis, including care home staff, therapists, nurses, GPs & Advanced Nurse Practitioners
Have excellent communication & negotiation skills
Have a keen interest in speaking with patients & improving their experience within Intermediate Care
Be competent with IT systems; experience of NHS electronic patient record systems would be advantageous
You will work across the Barnsley area therefore being a car driver & having access to a car is essential
The role is 30 hours per week Monday to Friday & could include up to 8pm; however, you may be expected to work some weekends. This may vary due to the needs of the service
The NRS delivers short-term, intensive rehabilitation in the community supporting people after hospital discharge or helping prevent avoidable admissions. Its a fast-evolving service with a strong focus on empowering patients to regain independence & confidence
