Job summary
37.5 hours per week
The role is to become a member
of the multi-disciplinary team working together to assess, review and offer
pharmacological and non-pharmacological treatments for people with Dementia, with
a view to obtaining a diagnosis if appropriate.
The client group will be
people referred to the service due to cognitive decline.
The purpose of the role is to
complete initial assessments for this client group to enable a diagnosis to be
made, to offer post diagnostic information and support to the person with
dementia and their carers.
Also, part of the role is
working with people whose presentation may have changed due to a variety of
reasons including Behavioural and Psychological Symptoms of Dementia.
The candidate should have a good
understanding of the symptoms associated with dementia and how this may affect
people and their day to day lives and the additional support that they and their
carers will require.
The candidate may be required
to work with people in their own homes and/or people in 24 hour care settings.
At

Main duties of the job
The successful candidate will
work within the multi -disciplinary team to assess people experiencing dementia
in both the community and care home settings.
Once a dementia diagnosis has
been formulated within the MDT, the candidate may be required to deliver that
diagnosis to the client: offering post diagnostic information relevant to that
individual and supporting the person with the diagnosis and their
families/carers.
Providing ongoing care
coordination for individuals on your caseload, including people subject to the
Care Programme Approach or equivalent clinical guidance under your care.
Offer pharmacological and
non-pharmacological treatments, review these and offer advice and support as
necessary. Providing ongoing review of individuals prescribed anti-psychotic
treatments in line with a local pathway.
To supervise junior members of
staff within their role.
Joint working/liaising with
statutory health and third sector services who may be involved in the clients
care.
Work with the whole
team to find new ways of working and innovative practice to meet the needs of
the client and their carers.

KEY RESULT AREAS:
The postholder will:
1.1 Care/service/function provision:
- Develop therapeutic relationships in which people using services are empowered and enabled to make informed choices about their health and healthcare.
- Participate in; the management of referrals, provision of clinical supervision to memory service nurse colleagues and junior clinical staff facilitating case review discussions when required.
- Provide comprehensive mental health and social care assessments and provide care, treatment and educational interventions within the bounds of the agreed protocol and/or pathway within the service.
- Review the efficacy of anti-dementia medication that has been prescribed for service users in line with the service pathway & protocol.
- Act as care co-ordinator for service users as required.
- Attend multidisciplinary meetings and care programme meetings as required.
- Ensure that carers needs are assessed and appropriate interventions or onward referral is provided depending on need.
- Communicate assessment outcomes sensitively to service users and relevant others. Inform the referring clinician of the assessment outcome and plan of care and treatment through provision of effective clinical reports.
- Promote health through provision of health education information and advice
- Following qualification as an independent non-medical prescriber, prescribe medication as an independent nurse prescriber within their own sphere of competence and the bounds of their approved prescribing status in accordance with the Trust non-medical prescribing policy.
- Promote patient choice and understanding when communicating assessment outcomes or prescribing medicines, including provision of high quality information both written and verbal to support this.
- Be responsible for ensuring the security and safe handling of FP10 prescription forms and to report any loss or theft as per Trust guidelines
1.2 Working as part of a team:
- Recognise the limits of their own expertise and (in line with the local protocol/ pathway for assessment of suspected dementia), request additional assessments from more senior multi-professional colleagues when required.
- Access regular caseload and clinical supervision (as identified within the local service protocol/pathway), to summarise, present and discuss assessments and therapeutic interventions that have been completed with more senior multi-professional colleagues when required.
- Utilise an appropriate range of multi-agency and inter-professional resources in their work, liaising effectively with other teams and agencies according to individual service user/ carer need.
- Work collaboratively in partnership with service users and health & social care professionals to provide specific interventions which are evidence based and appropriate with regard to context and culture.
- Liaise effectively with colleagues from primary care and non- memory specialist secondary services, providing information and advice where appropriate on memory assessment and interventions.
- Actively support the teams development of prescribing protocols, policies etc. as required.
- Ensure that the patients GP is notified of any changes to that patients care or treatment, including commencement of any new medication in accordance with Trust policy.
- In the event of adverse drug reaction, to be accountable for ensuring that the GP/Consultant is advised accordingly.
- Appropriately liaise with staff from external health, social care and third sector organisations.
