Junior Community Sister/Charge Nurse

37.5 hours per week (internal only)

We are looking for an experienced & dynamic Junior Community Sister/Charge Nurse to join our Neighbourhood Nursing Team. This is a pivotal role, working alongside the District Nurse & Team Leader to provide clinical leadership, deliver high-quality care & support the proactive management of patients with complex health needs.

The successful applicant will have experience of working within a community nursing or crisis response role & thrive in a multi-disciplinary team environment including nursing specialists, AHPs, primary care & acute colleagues

The role will primarily support the planned nursing pathway on complex patient management, caseload management & providing clinical support for staff within Neighbourhood teams to drive our pathways forward. You will also have the skills to work across our crisis pathways.

The successful applicant will be expected to work across a seven-day week shift pattern including weekends & bank holidays

Main duties of the job

Be a registered General Nurse & must be willing to complete the Nurse Prescriber Course

Take responsibility for a defined cohort of patients with complex or higher acuity needs within the District Nursing caseload

Use your extended clinical skills & knowledge to assess, plan, & implement evidence-based treatments that improve health outcomes

Collaborate with the multi-disciplinary team to deliver person-centred care that promotes independence & self-management

Lead by example, supporting colleagues & contributing to the proactive management of patients in the community

Support the District Nurse/Community Matron in managing higher acuity patients & deputise when required

Support staff development through supervision, mentorship, & preceptorship programmes
