Job responsibilities

JOB SUMMARY

1. To provide clinical nursing to the highest standard within the ward maintaining a clean safe environment

2. To provide and maintain a high quality of individualised patient care by effectively using all available resources

3. To assess, implement, monitor and evaluate the care and treatment of all inpatients in partnership with the patient and their relatives/carers

4. To establish good working relationships with all members of the multidisciplinary team..

5. Designated to take charge regularly of the ward, which provides rehabilitation, respite and continuing care in the absence of the Sister/Charge Nurse.

6. To supervise and delegate care according to other team members (and your own) skills and competences in the absence of the sister/charge nurse

7. Identify potential problems and report them to sister/manager.

8. Participate in the teaching, education and training of all ward based staff including student nurses, post registration students, NVQ students, patients and relatives/carers

9. Required on an ad hoc basis to be senior nurse bleep holder. (For cardiac arrest and absence reporting)

KEY RESULT AREAS:

CORE DUTIES RESPONSIBILITIES:

1. To support ward sister/manager by contributing to the day to day running of the ward to ensure high standards of quality care is delivered which adequately meet the expectations of patients and carers and to ensure privacy and dignity is maintained

1. To carry out assessment of patient care needs and the planning and implementation of programme of care including discharge and to monitor and evaluate care as delegated. Respecting and involving families and carers appropriately.
2. Reporting at handover any patient changes, to ensure continuity of care.
3. To assist in the education of relatives / carers to enable them to develop relevant skills they would need to carry out on discharge
4. To maintain patient confidentiality at all times.
5. In absence of Ward Sister/Charge Nurse to delegate work appropriately to team members, participate in ward rounds, MDTs and Best Interest Meeting
6. To report incidents, accidents, and complaints promptly to ensure corrective action taken. To be aware of and implement Health and Safety within the workplace in order to maintain a safe working environment
7. To be aware of and implement all Trust and departmental organisation and clinical policies and procedures including infection control
8. To participate and assist in the development and introduction of research/ evidence based practice, innovations and NICE guidelines / standards and CQC outcomes
9. To take every reasonable training opportunity to improve and develop professional knowledge and skills
10. To attend staff meetings and identified training (including Trustwide Induction)
11. To participate in the Trusts personal development/annual appraisal and clinical supervision programmes
12. To act as preceptor/mentor to student nurses and newly recruited staff and act as support to colleagues
13. Monitor and supervise the maintenance of accurate records having due regard to legal aspects including safeguarding, confidentiality and management information requirements.
14. To maintain accurate electronic and paper records as required within our organisational and legal framework
15. Assist with the control of supplies and equipment at ward level in order to ensure sufficient serviceable stocks are available for utilisation and remain within resources / Cost Improvement Programme (CiP) and budget allocation
16. Responsible for safe keeping of controlled drugs in line with Trust policies and Procedures
17. To ensure safe administration of all medicines in line with NMC guidelines and Trust policies and Procedures.
18. To promote the health and wellbeing of service users, in conjunction with other relevant services maximising partnership working with internal and external services

1. To participate in the planning and development of our service having an awareness of Trust Corporate Objectives, Strategy and Business Plan
2. To undertake such other duties as may be determined from time to time within the general scope of the post.

Qualifications

Essential

• 1st level Registered Nurse.
• Current NMC registration.

Training

Essential

• Evidence of CPD.

Desirable

• Experience of mentoring/supporting learners or ability to demonstrate the skills required for effective mentoring.
• Neuro Courses.
• Stroke Care Course.

Special Knowledge/Skills

Essential

• Ability to communicate verbally at all levels and possess good written skills in line with record keeping standards.
• Leadership and influencing skills.
• Must have high clinical standards when dealing with patients.
• Ability to teach, mentor, and act as a role model.
• Evidence of continuous professional development (CPD).
• Knowledge of Health & Safety issues including risk management and safeguarding.
• Knowledge of Infection Control.

• Basic IT skills.
• Knowledge of current issues affecting health and social care.

Experience

Essential

• Experience of working with stroke / neurological patients in a nursing / rehabilitation setting.
• Experience of working within a multi disciplinary team.
