This is an exciting opportunity to join a well established, hard working and welcoming team in the East sector of Barnsley. As an Enhanced Mental Health Team (EMHT) we are a group of specially trained people who work together in order to offer support to service users who experience complex mental health difficulties.
We work flexibly with service users as a multidisciplinary team depending on their level of need and coordinate services and care to work towards mental health stability and recovery.

Main duties of the job
The role will include completing full and comprehensive mental health assessment for patients on referral to this service, this includes identifying mental health needs, wider social needs and risk assessment and management plans.
We utilise the FACT model in patient care where appropriate and you will be responsible, when shift coordinator and as a care coordinator for being responsive to patients under this model of care and working as part of the MDT to meet these service users needs.
Create, undertake and review comprehensive patient care plans with the input of service users which promote engagement, stability, recovery and risk management. Work alongside other professionals in delivered of planned care, evaluate and review as appropriate.
Document care delivery to a high standard in line with professional standards, local and national guidance.
Supports the person and carers to promote health and well-being, educate around harmful behaviours and signpost to relevant services.
Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
Responsible for student education when allocated to the team and to contribute to the preceptorship and education of newly qualified staff where appropriate.
Administration of depot medication in the community.
