opportunity has arisen for a Urology/Continence Nurse Specialist (Band 6) to
join a well established community Continence and Urology team within the
Barnsley area.
We are looking
for a dynamic, motivated and enthusiastic Nurse with substantial Continence or Urology
experience who can work autonomously and prioritise/organise workload ensuring
effective management skills.
Excellent
communication and interpersonal skills, both written and verbal are essential
along with the ability to utilise electronic IT systems as part of your daily
work.
You will need
to be a car owner and driver with the ability to undertake travel in connection
with the duties of this post.
The post holder
will be expected to work flexibly as needed.

JOB SUMMARY
- The post-holder will support the Team Leader in the delivery of the Continence and Urology Service in community.
- To manage a defined clinical caseload, providing specialist support, education, and complex case management to people with continence and urological conditions and their families and carers.
- Work as part of a wider Neighbourhood Multi-disciplinary team of nursing, therapy, mental health, social care, and primary care staff.
- Liaise with primary and secondary care colleagues, providing education and support, thus developing and maintaining high quality evidence-based continence and urology care across Barnsley
KEY RESULT AREAS:
- Work autonomously assessing and managing own clinical caseload for all clients/patients within a designated neighbourhood (both clinic and home visits). Supporting patients with a range of continence and urological conditions.
- Use and apply specialised skills underpinned by advanced theoretical knowledge of professional practice in order to maintain professional competence and fitness to practice as a Continence and Urology Nurse Specialist
- To work as an specialist practitioner within the Continence and Urology Service demonstrating evidence-based knowledge, investigative and analytical skills and specialised assessment techniques to triage, assess and manage a range of complex presentations, taking into account physical, psychological and social care needs.
- Formulate and implement treatment plans based on current evidence and specialist clinical reasoning demonstrating a comprehensive understanding of treatments including the integration of the principles of self-management, enabling individuals to function independently by increasing their choice to remain in their own home/community, thereby reducing inappropriate hospital admissions.
- Utilise specialist clinical skills and draw upon evidence based practice as a source for giving and improving care when assessing, planning, implementing and evaluating programmes of care for individual clients.
- Enable people to receive an early and accurate diagnosis and receive the relevant information and support to manage their continence and/or urological condition.
- Participate in multi-disciplinary team meetings, contributing to decisions with regards to patient care.
- Work alongside Primary Care GPs and Practice Nurses and in managing patients within the respective Primary Care Network.
- Work in collaboration with the continence and urology and medical teams within secondary care to help facilitate the early supported discharge of patients following hospital treatment, to support the provision of seamless care.
- Manage referrals to the service from medical, nursing staff and other sources and ensure patients are seen within the required timescales based upon clinical need. This will include patients requiring assessment and support, those at risk of hospital admission and End of Life patients.
- Play an active role to initiate, plan and implement induction, training and education of junior team members, the broader community nursing teams and students.
- Ensure appropriate supervision and caseload support of junior members of the team
- Understand the requirements and implications for service delivery of National Standards eg NICE guidelines, National Service Frameworks where applicable
- Lead on the development and delivery of specialised education programmes to own and other professionals to enable best practice in the care and treatment of individuals with continence and/or urology conditions
- Adhere to and as appropriate contribute to the development and review of policies, procedures and standards relating to the service; using an evidence based approach.
- Work within and as appropriate contribute to the clinical governance framework to maintain consistently high standards of care, working from an evidence base or current best practice.
- Participate in developing evidence based policies and clinical guidelines related to continence and urology with collaboration of the clinical lead.
- Ensure complete, accurate and up to date records are maintained in accordance with NMC guidelines and trust policy. Record all client contacts and activities, and input data into appropriate IT system in a timely manner in accordance with PCT protocols.
- Adhere to the NMC scope of professional practice and trust policies and procedures; ensuring professional competency and development by updating skills and knowledge; attending all mandatory training.
Communication with others:
Communicate effectively, empathically and in a cultural sensitive manner with patients, representatives, carers and their families, interpreters will be used effectively where the need is identified.
Communicate effectively with all members of the Continence and Urology Service; providing information as necessary to provide a cohesive and effective service.
Communicate effectively with SWYPFT GP practices and health professionals in other areas.
Develop and maintain effective appropriate lines of communication with all health care staff working in partnership with other members of the primary health care team and with statutory and voluntary agencies and dealing with members of the general public.
