Job summary

We are looking to appoint Rehabilitation Support Workers to join our Neighbourhood Rehabilitation Service (NRS) which encompasses the Intermediate Care team & Long-Term Condition Pathway

The service provides specialist community therapy assessment & treatment to improve health & independence

You will work as part of a multi-disciplinary team alongside Physiotherapists & Occupational Therapists

You will have a desire to work in the NHS be motivated & flexible as you will work a shift pattern, including weekends & bank holidays

You must have A-D/grade 3-9 GCSEs or equivalent in English & Maths

If you do not hold an appropriate level 2 (or above) in health & social care you will be supported to achieve level 2 qualification this will include time at college for formal study

If you are recruited as an apprentice & you have GSCE grade D or 3 you will be required to undertake level 2 Functional Skills as part of your college attendance

At the time of advertising, this role does not meet the minimum requirements set by UK Visas and Immigration to sponsor candidates to work in the UK. For this reason, unfortunately we are unable to sponsor anyone on a skilled worker visa for this role for the time being.

Main duties of the job

As a Rehabilitation Support Worker you will

Promote independence & well-being, help empower individuals

Assist in initial assessment & development/implementation of care plans

Assist patients with personal care

Deliver & issue prescribed equipment under the direction of clinical staff

Support patients with exercises

Collaborate with healthcare professionals, work in close liaison with the MDT

Communicate effectively with patients & their families

Participate in training, education & clinical supervision

The use of a vehicle to travel in connection with the duties of the post is essential.
