Main duties of the job
The successful candidate will manage a caseload of around 25 people, they will be responsible for arranging care, liaison with other professionals and agencies, manage and review care plans and risk assessments ensuring these are done collaboratively in a recovery focused manner utilising the flexible assertive community treatment model.
They will also ensure CPA reviews are completed in a timely manner and to a high standard and are inclusive, keeping the person and their carers in the centre of all we do.
They will work as a shift coordinator on a rota system which will require them to delegate and prioritise work, oversee and facilitate the daily morning meetings, managing the team diary and carry out any urgent visits.
The successful candidate will be an efficient organiser, managing their workload and be flexible to meet the needs of service. They will manage their own requirements for supervision in line with their professional registration.
