Following retirement of the current post holder and in line, we are pleased to be able invite applications for a Advanced Clinical Pharmacist post.
As a registered pharmacist with suitable clinical experience, you will be responsible for leading and delivering the pharmacy service to our rehabilitation wards, community teams and Barnsley Hospice. There may be an expectation to provide cover to some MH services.
Our team provides clinical pharmacy and medicines supply services to Stroke and Neuro rehabilitation, Mental Health, Learning Disability and Community services across the Trust. The post holder will also be the Lead Pharmacist for Barnsley Hospice. Pharmacy team are valued members of the multidisciplinary teams. Our services are developing into new areas and we are introducing an electronic prescribing and medicines administration system
The Barnsley team is a small team that is predominantly based at our Kendray Hospital site but we also provide services to a number of community and community mental health services. Travel will also be required to Barnsley Hospice
Main duties of the job
You
will lead and deliver pharmacy services to the Barnsley locality of the Trust
(SWYPFT).With a focus on rehabilitation and palliative care
You
will provide specialist advice on pharmaceutical matters, undertaking risk
management, clinical judgement and ensuring compliance with Trust procedures,
national guidance and legislation. This includes provision of care to
individual service users, carers and service .
You
will lead on clinical pharmacy services for patients within the locality for
the specialisms above.
The post will be based within at Kendray hospital but the applicant must
be adaptable to the needs of the pharmacy service and may be required to travel
to other parts of the locality as part of their role.
We aim to provide a pharmacy and medicines optimisation service which
works collaboratively and compassionately with all services and service users
to provide integrated, high quality, person centered pharmacy care and services
to all.
You will take part in the trust wide on- call rota currently
approximately 1 week in 14.

Provison of professional responsibilities of a pharmacist supervising the dispensing of prescriptions at Fieldhead Pharmacy whilst fulfilling the dispensary commitment.
Participation in the Saturday, Bank Holiday and On-call Rotas
In conjunction with the pharmacy team, develop and implement safe and effective policies and procedures in line with New Ways of Working
To provide management to allocated staff including appraisal , development and clinical supervision
To be involved in the development of new services as allocted by pharmacy management team
Develop and participate in Trust-wide medicines management audits and attend allocated governenace groups to represent pharmacy.
KEY RESULT AREAS:
- To be responsible for the clinical pharmacy and prescription monitoring at allocated clinical teams . Be a specialist medication referral point for these teams.
Performance indicators
- Medicines reconciliation targets for inpatient areas
- Ensure all service users have access to clinical pharmacy services
- Screening for all service users admitted to Barnsley hospice
- Develop and contribute to patient focused services to allocated teams , including service user groups.
- Provide appropriate and timely verbal and written information on all aspects of medication usage to service users, carers and healthcare professionals within SWYPFT and Barnsley Hospice.
- Participate in in-house (incl Hospice) training for pharmacy staff and healthcare professionals , both attending and delivering training.
- Participate in the training of pre-registration pharmacy graduates and mental health pharmacy diploma students.
- Participate in the development of the medicines management clinical audit programme in particular the POMH audits and ensure roll out to allocated units.
- Participate in research and development as required for the development of the clinical pharmacy services.
- To cover for other senior pharmacists in their absence.
- If required due to sickness or other absence to work in other localities of the organisation.
- To carry out CPD in line with the GPhC requirements.
- To maintain and demonstrate a good clinical pharmacist knowledge.
- To work within the policies and procedures of the South West Yorkshire Partnership NHS Foundation Trust and also to ensure compliance with the pharmacy policies and procedures from other organisations when working in partnership.
