arisen for Registered Nurses to work within Critical Care at Pinderfields
Hospital (Mid Yorkshire Hospitals NHS Trust).
The Critical Care unit has twenty beds including Level 3 ICU beds, Level 2 HDU beds and two Level 3 Intensive Care
Burns beds.
We are looking for enthusiastic,
well-motivated and caring individuals, to work within the multidisciplinary
team in providing high standards of patient care, and the maintenance of a safe
working environment.
The successful candidates will
have the ability to plan, assess, implement and evaluate patient care. Good
interpersonal skills are essential, along with a willingness to learn; the
ability to work effectively in a team; to be self-motivated and to have a
positive attitude towards change and to be able to adapt to the change quickly.
Candidates should have recent NHS experience; Critical Care experience is
desirable but not essential. Full training and competency assessment will be
provided.
Main duties of the job
The post is for a qualified nurse who wants to further their experience and skills in nursing. The post holder will be responsible for the assessment of care needs, the development of programmes of care and their implementation and evaluation.
- Monitor and evaluate patient progress.
- Administer treatments.
- Identify changes in a patient's condition and react accordingly.
- Maintain patient records.
- Complete paperwork necessary for patient transfer out of the intensive care unit.
- Maintain patient safety -Assess, plan, implement and evaluate patient
care using evidence to rationalise care for both conscious and unconscious
patients, under the indirect supervision of a senior nurse. As the nurse
becomes more experience demonstrates specialist knowledge and skills in this
process. Able to make ongoing evaluation of the effectiveness of interventions
in meeting prior agreed goals and make necessary modifications to the care
plan.

Receive training and be assessed as clinically
competent in achieving core critical care competencies following the KSF for
this band. Developing further skills to allow the nurse to provide all care for
any general intensive/high dependency patients admitted to the individuals
area of work. As experience progresses demonstrate ability to competently
provide care for critically ill patients presenting with a wide range of
conditions and treatment requirements, initiating and promoting excellence in
clinical practices as a role model to junior staff. Evidence provided by

Prepare bed areas for admission of critically
ill patients ensuring all equipment is available, correctly assembled and
functional.
Assess, plan, implement and evaluate patient
care using evidence to rationalise care for both conscious and unconscious
patients, under the indirect supervision of a senior nurse. As the nurse
becomes more experience demonstrates specialist knowledge and skills in this process.
Able to make ongoing evaluation of the effectiveness of interventions in
meeting prior agreed goals and make necessary modifications to the care plan.
Maintain a culture which recognises and works in
partnership with patients and their carers involving them in all decisions
relating to their care delivery. Act as the patients advocate where
appropriate.
Work in collaboration with other professionals
to promote seamless patient care delivery. Process diagnostic tests and obtain
samples as requested by medical staff e.g. blood taking. Report results when
available to appropriate staff.
Responsible for own and whole departments
requirement to reduce hospital acquired infection and cross infection through
the implementation of good practice e.g. effective hand washing; daily damp
dusting of all bedside equipment; using protective clothing appropriately;
rigorously applying standard (universal) precautions to minimise the
transmission of infection.
Receive training, and be assessed as clinically
competent in using all equipment available on the critical care area developing
this knowledge to enable them to be able to train others in its use e.g. ventilator support; invasive pressure
monitoring; intravenous infusion devices; near patient testing; dynamic
mattress systems.
Constantly observe patients haemodynamic and ventilator
status, accurately record observations and take appropriate actions according
to competence. Report any change in the patients condition to the nurse in
charge.
Safely care for patients with multiple infusion
devices, maintaining accurate fluid balance.
Ensure all patients receive an appropriate
nutritional intake to meet their requirements, including oral food and fluids,
enteral and parenteral feeding regimes.
Consistently use correct moving and handling
techniques and appropriate equipment when moving or handling patients, stock
and equipment which may be soiled or infected.
Be able to assess patients level of
consciousness, utilising appropriate tools. Report changes as appropriate. Able
to safely care for aggressive and/or confused patients, maintaining a safe
environment at all times for patients, staff and visitors.
Assist medical staff in the insertion of
invasive devices e.g. central lines, endotracheal tube; and performing
diagnostic procedures e.g. bronchoscopy, angiography. Able to prepare all
equipment and safely care for the patient during and after the procedure
initially with appropriate supervision of senior nursing staff, as skills and
knowledge develop to practice autonomously without supervision.
Respond appropriately in emergency situations to
support the multidisciplinary team, actively participating in cardio pulmonary
resuscitation.
Sensitively care for dying patients and their
families, provide access to information and support. To be involved in organ
donation processes as appropriate.
Safely prepare patients for discharge.
Communicate verbally and in writing clear relevant information to receiving
wards, specialist clinical teams, patients and carers, and other disciplines
e.g. Social Work Department.
Through training and development able to
transfer all levels of patients (dependent upon area of practice) both inter
and intra hospital.
Actively involved in internal rotation within
the critical care areas to develop clinical knowledge and skills and ensure a
safe patient and staff environment in times of peak activity.
