Job summary
An exciting opportunity has arisen for three (3) Band 4 FIT Coordinators to join the Cancer Team at the Mid Yorkshire Hospitals Teaching Trust for a fixed term of twelve (12) months.
The post holders will be the main point of contact for patients referred to the Trust by GPs for a Faecal Immunochemical Test (FIT). They will be central to the effective communication between patients, the healthcare professional multi-disciplinary team and Primary Care. They will be required to monitor referrals prior to, and after a 2-week wait referral.
The role requires the post holders to be self-driven, highly organised and have excellent verbal and written communication skills. The role will involve liaising with patients, their relatives and clinical and administrative teams, within both the Trust and Primary Care to coordinate FIT results, send out test kits and supporting patients to return completed investigations. They will also provide input in meetings to provide information to support the achievement of waiting times standards and ensure shared learning from any pathway breaches.
The post holder is responsible for proactively organising the patient's pathway in relation to the FIT test to ensure patients are referred quickly.
Previous experience of working within a healthcare or customer service setting, awareness of data confidentiality and cancer waiting times and proficiency with software programmes such as Microsoft and email systems will be advantageous.
Main duties of the job
Act as the key point of contact for patients, providing first-line support to patients on their pathway, signposting them on to other services as necessary.
Ensure that test results, clinic letters and notes are available and communicated to the clinical team at the earliest opportunity to inform the next steps in the pathway.
Access the pathology report on a daily basis to check any FIT referrals that have come in.
Create entries on the Cancer Information System for each referral and ensure tracking status is set.
Create and maintain a spreadsheet to keep track of FIT tests referred in.
Pull tracking lists daily and filter to those patients awaiting FIT tests
Update referrals on the Cancer Information System with FIT results and values
Use SystmOne to obtain information to create a 2-week wait referral on the ERS system if FIT results are positive or end tracking on the Cancer Information system if results are negative.
Contact patients to inform them of a positive FIT result and explain next steps.
Send out and collect patient surveys.
Track patients who have been referred on for triage.
Ensure attendance at FIT project meetings and provide updates as needed.

collected and recorded for each cancer site responsible for and adhere to
relevant deadlines
Ensure site specific data is collected on
relevant databases, using identified data collection processes, in agreement
with clinical teams
To provide basic analysis of the data for
Clinicians and the Cancer Data Manager and assist in any audits when required
To develop new and maintain
existing contacts with the appropriate departments within the Trust and the
Integrated Care Board (ICB). This may involve site visits.
To liaise with members of staff at all levels,
e.g. Consultants and other health professionals in the Trust and also with the
tertiary centres
To ensure accurate entry of data into Info flex
allowing data transfer on agreed deadlines and in accordance with National data
submissions. For example COSD.
Tracking of patients with targets set to predict
when next data collection is required, and to check if all investigations are
underway
To provide cover for other Care Navigators and
the Straight to Test Tracker in times of absence for example annual leave,
sickness absence
Undertake any other ad hoc duties deemed
relevant to the post and grade
If needed, partake in
supporting the implementation of the Capsule Colonoscopy project,
